Key Responsibilities
Core responsibilities include:
Worker Registration &amp; Badging: Proactively monitor and manage access control compliance for everyone who comes in and out of the site. Work closely with the customer and Sitemetric team to understand and uphold site security requirements.
Badging Compliance: Ensure that all personnel coming into the site have their badge and are properly scanned in to verify their identity and keep consistent worker headcount records for our customers.
Parking Management: Provide parking passes to appropriate personnel and monitor parking pass compliance across designated parking areas.
Visitor and Delivery Management: Receive and document all visitors and deliveries that access and leave the site, keeping consistent written documentation for the customer’s records.
Customer and Worker Support: Provide support in all areas of access control ranging from worker registration and badging, tech troubleshooting, visitor and delivery logs, and parking management.
Ad hoc responsibilities: Assist with ad-hoc access control responsibilities to support our customers on one-time and ongoing access control related needs.
Qualifications
REQUIRED

<li>2+ years of customer service experience in similar areas like coffee baristas, nurses, childcare, or similar areas with an emphasis on serving people</li>
<li>Ability to establish and maintain effective working relationships with internal and external customers</li>
<li>Comfortable downloading, navigating and using mobile apps</li>

Job purpose
Reporting to the Program Manager for Cancer Care Health Equity, the Community Health Worker for Cancer Care (Lung Cancer Screening) is a community ambassador, educator, and navigator for cancer prevention at Fair Haven Community Health Care (FHCHC). The role coordinates FHCHC’s patient-facing efforts at preventing cancers of all types, including breast, cervical, colorectal, and lung. The role effectively communicates about cancer prevention in ways that resonate with communities served by FHCHC.
Duties and responsibilities
<ul>
<li>The Community Health Worker for Cancer Care (Lung Cancer Screening) will perform direct outreach to patients requiring cancer screening to provide education about the risk factors and the importance of early detection. Typical duties include but are not limited to:</li>
<li>Assist patients in navigating the healthcare system, including scheduling appointments, obtaining referrals and navigating insurance coverage</li>
<li>Coordinate care between primary care providers, specialists, and other healthcare facilities</li>
<li>Establish and maintain relationships with community-based organizations that provide social services, including transportation, food assistance, housing and childcare</li>
<li>Screen for and address the social determinants of health that may prevent access to cancer screening among FHCHC patients</li>
<li>Collect and maintain accurate patient data, including demographics, screening status and outcomes</li>
<li>Contribute to the development and evaluation of program processes and protocols</li>
<li>Participate in the development and implementation of program strategies to increase cancer screening rates</li>
<li>Collaborate with the marketing team to design and implement social media and other media campaigns for cancer screening education</li>

Qualifications
Associate’s degree, relevant experience and demonstrated knowledge of communities in New Haven is required. &nbsp;&nbsp;A Bachelor’s degree preferred.&nbsp; Bilingual English and Spanish and proficiency with Microsoft office is required. Community Health Worker certification and experience in health care with electronic medical records systems are highly desirable.

Direct Service Staff Overview:

The Program Manager is a mid-level management position under the direct supervision of the Program Director. The Program Manager stablishes clinical systems and standards of care. PROGRAM DESCRIPTION: 
The San Bernardino Wrap program is a community-based, family-centered, needs-driven program designed to maintain children with emotional disturbances in the community at the lowest level of care possible. The goal of the program is to reduce the risk of out-of-home placement and recidivism by brining individuals, agencies and the community together as the decision-making team with the central focus being to meet the needs of the child and family. Services assist in supporting the mental health needs of children and their families, allowing safe and appropriate care, and promoting coordination and collaboration in care planning efforts with the Children and Family Team (CFT) and other child-serving agencies. 
 
GENERAL REQUIREMENTS:
Education/Experience
