Job Description

Job Description 
The RN Case Manager reports directly to the Clinical Supervisor and is responsible for care coordination of clinical staff and patient/family clientele. Responsible for admissions and recertification assessments, including supporting documentation, as assigned. This position ensures compliance with MGA policies, State policies and procedures, and policies of accrediting agencies, as well as responsible for maintaining compliance with all reimbursement guidelines and maintaining patient confidentiality/HIPPA guidelines. Furthermore, individual will participate in program development, implementation, as well as collaborating with Client Service Managers to acquire/develop new relationships with clientele.&nbsp;
Duties and Responsibilities&nbsp;&nbsp;&nbsp;

 Additional Information

Under the leadership of the Assistant Administrator, the Wound Care Clinical Nurse Manager is an active member of the management team that delivers professional wound care services and support that is consistent with the strategic vision, goals, philosophy, and direction of Wound Center/department and CRHS. The Clinical Nurse Manager is responsible for the coordination of clinical activities of the outpatient wound care clinic, the use of organizational skills, leadership, planning, implementing, evaluating, and providing patient care using hospital and nursing standards. The Clinical Nurse Manager is a professional nurse who utilizes the nursing process to provide direct patient care for people, acute and chronic wounds, drains, tubes, fistulas, pressure ulcers and with fecal and urinary diversions. The Clinical Nurse Manager has an educative role (staff in-services, patient teaching and community education), advocacy role, as well as research and administrative duties. This position works in collaboration with the Program Director and Medical Director to meet the Center’s program objectives to help ensure optimal healing outcomes and patient satisfaction.
Basic Qualifications:
· Education:
Requires a Bachelor of Science in Nursing from an accredited school of nursing.
· Experience:
