Summary of Position:&nbsp;The Medical Secretary is directly responsible for all office operations necessary to ensure that medical records are current, complete, and in compliance with EverHeart Hospice policy. He/ she will ensure correct CPT and ICD codes are used for proper billing.

<li>Ensure current certification and re-certification forms are completed for each patient.</li>
<li>Maintain the current filing of all clinical records into medical records.</li>

<li>Assist the Finance Department with auditing medical records prior to billing, as requested.</li>
<li>Ensure the plan of care and/or orders are signed and returned by physicians in the time frames established in policy.</li>

<li>Mail and enter FEBS, FEHC, and current Care Surveys every month.</li>
<li>Maintain all patient logs.</li>
<li>Maintain the DME report and compare the DME to the patient chart, and make sure they are the same.</li>
<li>Prepare IDG list and notify nurses of patients who are up for review.</li>
<li>Fax IDG Summary to primary physicians.</li>
<li>Assist with the Hospice Aide schedule.</li>
<li>Make appropriate copies of patient information as requested (i.e., transfers, chart audits).</li>
<li>Assemble admission charts and transfer packets as needed.</li>
<li>Maintain charts and chart reviews.</li>
<li>Direct incoming calls, greet visitors, route telephone and faxed messages appropriately.</li>
<li>Perform opening and closing office duties as applicable.</li>
<li>Complete all documentation as required by the organization in compliance with Agency policy, accreditation bodies, and governing requirements.</li>
<li>Gather supply needs from clinical staff, order supplies, and prepare supplies for distribution weekly.</li>
<li>General clerical duties to include, but not limited to, utilization of Microsoft Office products, photocopying, faxing, mailing, and filing.</li>
<li>Other duties as assigned.</li>

Education and Experience:

Responsible for entering information accurately in both the Medical Records and Patient Billing systems. Ensures completion and accuracy of all registration forms and documents and schedules follow-up visits into proper systems. Prints daily clinic schedules, prepares new patient information packets, answers telephone and triages calls/questions as appropriate. Collects physician practice authorizations, enter appropriate information in both registration/billing systems. Instructs patients to obtain referrals as needed. Assists with follow up directly with referring physician office as needed. Explains general policies related to registration, co-payments, referrals, pre-authorizations, and deductibles as well as policies related to physician billing and different specialty services within UMSJMG.
