<li>Performs new patient thorough work-up (x-rays, vitals, new patient packet explanation).</li>
<li>Adds medical history on all new patients and updates all established patients’ history in accurately.</li>
<li>Performs accurate and complete room set-up for physician.</li>
<li>Ensures that all patients medication is updated before physician visit.</li>
<li>Handles set-up for in office surgical procedures.</li>
<li>Ensures appropriate consent form is completed correctly avoiding the use of too many medical abbreviations within the consent forms.</li>
<li>Scribe as assigned per physician office flow.</li>
<li>Prepares for seeing patients the following day, including patient confirmation for next day visit.</li>
<li>Completes patient medical record requests when necessary.</li>
<li>Assists in the orientation and training of new personnel.</li>
<li>Ensures proper hand off of responsibilities once their task is completed.</li>

Join our dynamic healthcare group as a Post-Acute Care Medical Assistant and serve as a primary liaison for patients in our innovative care model that focuses on safe transitions from hospital to assignments. Job Summary
We are seeking a dedicated and detail-oriented Vital Monitoring Specialist to oversee all aspects of vital signs monitoring in a nursing home setting. This role ensures the consistent and accurate monitoring of residents' health metrics, facilitates timely medical interventions, and contributes to a safe and high-quality care environment.
Responsibilities
<ul>
<li>Vital Signs Monitoring:</li>
<ul>
<li>Regularly monitor residents' vital signs, including blood pressure, heart rate, temperature, oxygen saturation, and respiratory rate</li>
<li>Ensure proper use, calibration, and maintenance of monitoring equipment</li>
</ul>
<li>Data Recording and Analysis:</li>
<ul>
<li>Accurately record and update vital signs in residents' medical records</li>
<li>Analyze trends in vital data to detect potential health issues</li>
<li>Generate reports for clinical teams to support proactive decision-making</li>
</ul>
<li>Facilitate Interventions:</li>
<ul>
<li>Promptly notify nursing staff and physicians of abnormal readings or health concerns</li>
<li>Assist in emergency interventions, including alerting relevant medical personnel and preparing necessary equipment</li>
<li>Coordinate with clinical staff to adjust care plans based on monitored data</li>
<li>When needed, facilitate a Telehealth visit with a provider </li>
</ul>
<li>Resident and Staff Communication:</li>
<ul>
<li>Communicate vital signs results and health updates to residents and families as needed</li>
<li>Provide guidance and training to staff on proper monitoring techniques and equipment usage</li>
</ul>
<li>Compliance and Quality Assurance:</li>
<ul>
<li>Adhere to regulatory guidelines and facility protocols related to resident monitoring</li>
<li>Participate in quality improvement initiatives to enhance monitoring and intervention processes</li>
</ul>
</ul>
Requirements
<ul>
<li>Active Medical Assistant Certification</li>
<li>Prior experience in vital signs monitoring or a similar healthcare role, preferably in a long-term care or nursing home setting</li>
<li>Proficiency in using monitoring equipment and electronic health record (EHR) systems</li>
<li>Strong attention to detail and ability to identify irregularities</li>
<li>Excellent communication and teamwork skills</li>
<li>Ability to remain calm under pressure and act swiftly in emergencies.</li>
