Job Description
The family practice clinic provider focuses on utilizing a holistic approach to patient care with emphasis on health promotion &amp; patient education/counseling as well as supporting direct care staff through education and mentorship with the shared goal of providing exceptional patient care to a mixed population of community patients in the rural health clinic.
 
Specific Tasks include

Community Youth Services is recruiting for Psychiatric Medication Provider, providing in-person and/or telehealth with a monthly onsite visit. This position works with a treatment team that provides Intensive Outpatient Services to adolescents and young adults between the ages of 12 and 24 years in Thurston and Mason counties. Each participant in the Intensive Outpatient Program receives 24/7 crisis support from a team of therapists. CYS Behavioral Health Programs provide co-occurring treatment and are moving towards a medical integration model to provide whole-person care. This position provides outpatient psychiatric services, including staff consultation, psychiatric diagnostic medication evaluations, medication management, and psychoeducation for participants in the behavioral health programs.
The position will work closely on an integrated team with therapists, wraparound care coordinators, and peer counselors in alignment with the principles of our Evidence and Research-Based Practice models as well as agency best practices such as Trauma-Informed Care and Positive Youth Development. This position will work with a diverse youth population and their families in alignment with a youth/family-driven treatment, harm-reduction, prevention-focused, and holistic care philosophy. This position can allow for a combination of virtual and in-person services to be provided (with a preference for in-person).
 
Pay range of $100,000 to $120,000 DOE (Exempt) This position is part-time (20 hours a week) and will provide services in-person and virtually.

Required:

The family practice clinic provider focuses on utilizing a holistic approach to patient care with emphasis on health promotion &amp; patient education/counseling as well as supporting direct care staff through education and mentorship with the shared goal of providing exceptional patient care to a mixed population of community patients in the rural health clinic.
 
Specific Tasks include
<ul>
<li>Thorough medical history/exam and documenting</li>
<li>Comprehensive wellness and episodic primary care clinic visits</li>
<li>Prescribe medication, DME, &amp; treatments as appropriate</li>
<li>Order and interpret laboratory and other diagnostic tests as appropriate</li>
<li>Instruct patients on selected therapeutic and diagnostic procedures, consultation, and laboratory examinations relevant to their case</li>
<li>Educate and counsel patient and family members concerning health promotion, self-care skills, disease prevention, and treatment options in coordination with other healthcare providers</li>
<li>Participate in periodic meetings that enhance an interdisciplinary approach to primary health care</li>
<li>Identify quality improvement issues with respect to improving patient care</li>
<li>Maintain a current level of knowledge relative to professional medical practice</li>
<li>Educate and act as a resource to ancillary staff</li>
<li>Request for specialty referrals w/consultations where applicable</li>

SKILLS/KNOWLEDGE/ABILITIES:
In collaboration with supervising physician, duties may include, but are not limited to the following: 
<ul>
<li>Identify health status of patients by taking medical history, performing exam, and initiating and evaluating screening and diagnostic procedures.</li>
<li>Provide services in accordance with Standard procedures.</li>
<li>Determine which patient to refer to the physician for further diagnostics and treatment.</li>
<li>Plan and assume responsibility with physician for ongoing health maintenance and clinical management of patients in stable phases of acute and chronic illnesses.</li>
<li>Evaluate patients emotional behavior, counsel, and instruct individuals and families regarding domestic and health related problems.</li>
<li>Initiate orders for diagnostic laboratory tests and x-rays. Interpret and review results. Review medication refills.</li>
<li>Assure that adequate patient health records are maintained and transferred when required.</li>
<li>Serve as clinical consultant to nursing and ancillary personnel.</li>
<li>Coordinate work activities with families, community agencies, and other members of the health care team.</li>
<li>Promote economies in service and lower patient costs whenever possible.</li>
<li>Participate in development and annual review of policies and procedures. Participate in periodic medical records review. </li>
