Care Manager – Social Work (Remote) MUST BE LMSW or LCSW (New York)
Location: Remote | Orientation in the Bronx
Schedule: Monday–Friday | 8:00 AM – 5:00 PM (1-hour lunch)
Type: Full-Time | Contract (LOA Coverage)
We are seeking an experienced Care Manager (Social Work) to support care coordination and quality management for a diverse patient population. This is a fully remote role following initial orientation, ideal for a clinically strong social worker with care management experience.
In this role, you will use your discipline-specific expertise to review field assessments and develop holistic, patient-centered care plans addressing all domains of care.
What You’ll Do
- Review clinical and psychosocial assessments completed by field staff
- Develop and coordinate comprehensive care plans across medical, behavioral, and social needs
- Collaborate with interdisciplinary teams including nursing, providers, and community resources
- Monitor participant progress and adjust care plans as needed
- Ensure quality of care and compliance with regulatory standards
- Maintain accurate and timely documentation
Must-Have Qualifications
- Licensed Social Worker (LMSW or LCSW) in New York
- Experience in care management, case management, or managed care
- Strong knowledge of community resources and care coordination
- Excellent documentation and communication skills
- Ability to work independently in a remote setting
Nice to Have
- Experience with MLTC, PACE, or managed care populations
- Familiarity with geriatric or complex care populations
- Bilingual skills (Spanish or other languages a plus)
Additional Details
- Remote role following Bronx-based orientation
- Monday–Friday schedule – no weekends
- Temporary assignment covering leave of absence (duration TBD)
