Primary responsibilities: To provide oversight and coordination for the Behavioral Health Services being provided throughout PACE SEMI. This position provides administrative and clinical oversight for the Behavioral Health Clinicians and service line. 
<ul>
<li>Provide coordination of services between the Behavioral Health Specialists, Psychiatrists, Pharmacists, Site Leadership and PIC</li>
<li>Formal clinical oversite and supervision of all Behavioral Health Specialists. </li>
<li>Assure collaboration, communication and support services are aligned at the centers for behavioral health services. </li>
<li>Partner with Center Directors and center clinical managers for oversight of behavioral health specialists as they provide service to participants and clinical teams at each center. </li>
<li>Ensure collaborative agreements are up to date, within legal scope of practice between BHS and PIC. This is to assist with collaboration within the care teams and for diagnostic clarity.</li>
<li>Development of partnerships with Master’s Programs with Behavioral Health Tracts for provision of internships with PACE (this provides a gateway to assist in recruiting BHS). </li>
<li>Conduct complex case review with Behavioral Health Specialists, develop and implement journal club for ongoing staff development. Identify high utilizers in the BH population for targeted case review.</li>
<li>Conduct monthly/bi-monthly meetings with BHS staff. </li>
<li>Collaborate with Behavioral Health Educator to design and develop on -boarding for all staff, competency evaluation of Behavioral Health Staff, on-boarding for Behavioral Staff, AIMS testing, mandatory reporting requirements, use of diagnostic screening tools, motivational interviewing, trauma informed care, Mental Health First Aid, differentiation between behaviors of dementia and behavioral health, appropriate referrals, and other programs identified that will improve Behavioral Health Care.</li>
<li>Identify and collaborate with others within PACE to establish community contracts for services not currently provided within PACE i.e. SUD, Suboxone, Methadone, Crisis Screening, 24 hour case management for BH patients, housing for BH/SUD patients. Examples would be Community Mental Health, Team Wellness, COPE, SHAR.</li>
<li>Review and update protocols for referrals, warm hand off after referrals, intake evaluations. Work with educator to ensure protocols are consistent across sites.</li>
<li>Collaborate with Director of Quality and Medical Director in the development of the Quality Plan as related to Behavioral Health Services.</li>
<li>Identifies metrics to be tracked related to Quality Plan:
<ul>
<li>i.e. ED visits for BH, SUD visits to ED, BHS staff turnover, number of significant incidents within programs with BH participants, participant attendance, time of referral to being seen by BHS/Psychiatrist after intake or return after hospitalization, completion of q 6 months BH vital signs completed, functioning of PACE participants on BH medications/psychotropics, retention rate of participants with a BH diagnosis, depression screening/treatment follow up, medication adherence, number of appropriate referrals to BHS. Develop a BH Dashboard with easy tracking of metrics. Accurately identify the number of participants with SUD- alcohol/drug/opiate usage, mild/moderate/severe BH diagnosis. </li>

<li>Work proactively with the intake team to help design and implement a plan of care for those with severe BH diagnoses. Ensure that there is a thorough initial assessment and follow-up appointments scheduled. Assist in the creation of Behavioral Plans for disruptive participants.</li>
<li>Provides and directs administrative support for the Psychiatrist including increase in client facing hours, documentation by BHS.</li>
<li>Reviews all orders for Neuropsych testing and works with ordering physicians to determine which are necessary.</li>
<li>Reviews/mentors/audits BHS documentation for clinical relevance, completeness, timeliness.</li>
<li>Works with site leaders to identify space for BHS to provide individual and group therapy, group education.</li>
<li>Oversees scheduling of BH specialists, site rotation for BHS.</li>
<li>Monitors case loads and productivity of BH specialists. Assists with determining if sites need additional or less BH coverage.</li>
<li>Ensures compliance with regulatory requirements</li>
<li>Collaborates with Site Leadership and Psychiatrist in providing evaluations of BH staff.</li>
<li>Collaborates on regular basis with Psychiatrist regarding Behavioral Health work, difficulties and challenges.</li>
<li>Ability to travel to all PACE SEMI centers</li>

Requirements:
Qualifications (knowledge, skills, abilities):

<li>Full Licensed Master’s prepared Social worker or Registered Professional Nurse, with current Michigan license, and Master's of Science in Nursing with a behavioral health nursing focus and nurse practitioner license.</li>

Position Summary
The&nbsp;MTM Pharmacist&nbsp;provides telephonic Medication Therapy Management (MTM) to high-risk members. This role directly supports better health outcomes through medication reconciliation, identification of medication-related problems, and coordination of clinical interventions. Pharmacists communicate with patients, caregivers, prescribers, nurses, and internal teams to optimize medication regimens based on nationally recognized guidelines. Training includes motivational interviewing and behavioral coaching techniques to support member adherence and outcomes.
Key Responsibilities
<ul>
<li>Conduct telephonic comprehensive medication reviews, including full medication reconciliation and clinical assessments.</li>
<li>Identify, document, and resolve medication-related problems.</li>
<li>Provide clear, health-literacy-appropriate written patient summaries.</li>
<li>Prepare prescriber-facing fax materials to support clinical interventions.</li>
<li>Build and maintain positive working relationships with patients, prescribers, nurses, and internal partners.</li>
<li>Perform additional duties as assigned by leadership.</li>
</ul>
Required Qualifications
<ul>
<li>Bachelor’s Degree in Pharmacy&nbsp;or&nbsp;PharmD</li>
<li>Active pharmacist license&nbsp;(good standing) in HI, TX, IL, FL, or PA</li>
<li>Verifiable&nbsp;High School Diploma or GED</li>
<li>Ability to maintain productivity and focus in a remote environment</li>
</ul>
Preferred Qualifications
<ul>
<li>Prior MTM experience (Outcomes MTM Connect highly preferred)</li>
<li>Previous call center experience</li>
<li>Strong organizational, communication, and interpersonal skills</li>

Position Summary
The Clinical Research Patient Navigator will play a central role in participant engagement, education, and retention for two Clinical Research Sites in the Los Angeles Metropolitan Area. The Navigator will serve as a bilingual advocate and liaison, with a particular focus on supporting the Hispanic patient population, who may face cultural and logistical barriers to participation. This hybrid position requires a consistent on-site presence, as well as remote work, to facilitate patient communication and chart review.

Key Responsibilities
Study Start-Up Resourcing and Planning
<ul>
<li>Tour all clinical facilities where participants will be seen.</li>
<li>Review study protocol and eligibility criteria; complete protocol and site SOP training.</li>
<li>Set up and maintain logs, trackers, data collection tools, and CTMS.</li>
<li>Assist with setting up HIPAA-compliant systems for hybrid work and access to medical records.</li>
<li>Review IRB-approved screening scripts to enhance patient engagement.</li>
<li>Contribute to the development of recruitment plans and outreach strategies.</li>
</ul>
Patient Discovery and Prescreening
<ul>
<li>Participate in chart review activities (patient assessments and medical records, especially for identifying potential patients or previous screen fails.</li>
<li>Build and implement a referral strategy with physicians and care providers in the community.</li>
<li>Support patient recruitment through mobile unit outreach and local community events.</li>
<li>Educate patients about liver biopsies and other procedures, addressing specific concerns or fears.</li>
<li>Serve as a consistent point of contact for Spanish-speaking patients and their families.</li>
<li>Ensure accurate and timely documentation of patient interactions, including communication outcomes and barriers identified.</li>
<li>Coordinate and confirm transportation to and from clinic visits.</li>
<li>Follow up with patients who received informed consent forms (ICFs) at home to confirm understanding and intent to participate.</li>
<li>Participate in chart review activities, especially for identifying potential patients or previous screen failures.</li>
<li>Coordinate visit logistics and reminders.</li>
</ul>
Enrollment and Ongoing Support
<ul>
<li>Support informed consent discussions as requested.</li>
<li>Serve as liaison for scheduling, reimbursements, and retention.</li>
<li>Build trust with participants through compassionate communication.</li>
<li>Track patient progress and address barriers to participation.</li>
<li>Patient retention per coordinator guidance.</li>
<li>Collaborate closely with the site coordinator to assist with retention and data tracking tasks as needed.</li>
</ul>
Site Collaboration
<ul>
<li>Work closely with site staff to ensure a seamless transfer of information.</li>
<li>Maintain trackers and reports on recruitment and patient engagement activities.</li>
<li>Participate in chart reviews and post-screen fail assessments.</li>
<li>Troubleshoot barriers to recruitment and retention and address obstacles impeding patient participation and experience.</li>
