Position Summary:
Under the direction supervision of the PACE Clinical Services Director, the PACE Referral Specialist is responsible for coordinating and processing medical, dental, and optical referrals and authorizations for a variety of settings that PACE participants utilize. This requires the understanding of universal codes such as Current Procedural Terms (CPT), Healthcare Common procedure Coding System (HCPCS), and Durable Medical Equipment, Prosthetics, Orthotics, ad Supplies (DMEPOS) in addition to International Classification of Diseases (ICD-10). These codes will meet the requirements for specialty services to ensure minimal delay in securing accessibility for participants.
Essential Functions:

<li>Receives and processes referrals and authorizations in a timely manner.</li>
<li>Coordinates care with primary care providers and their clinical coordinators in obtaining the necessary documentation needed for outsources care and diagnostics/radiology.</li>
<li>Interfaces with specialty offices to answer questions in regards to referral and authorization statuses.</li>
<li>Ability to navigate and utilize multiple functions from an electronic medical record platform in producing a comprehensive referral that will satisfy the needs of specialists.</li>
<li>Ability to use clinical and administrative judgement under regulations to determine best approach for both urgent and non-urgent patient care needs.</li>
<li>Maintain HIPAA standards and adhere to policies and procedures that govern the confidentiality of healthcare sensitive information.</li>
<li>Request additional medical records from specialty offices and clinic for further review by Utilization and Network Manager and/or PACE Medical Director as needed.</li>

POSITION SUMMARY:
The Information and Referral (I&amp;R) Specialist assists with CHASI's Community Food Pantry operations and conducts outreach to substance users to engage persons in services, provides a wide range of linkage &amp; navigation services, provides individual education to reduce health-related harm from injection and other drug use, conducts sterile syringe exchange, trains on opioid overdose reversal, and delivers HIV and hepatitis C (HCV) counseling and testing. The I&amp;R Specialist primarily works at the community access center to reach long-term and newer substance users, to partner with them as they set their own goals for improving their health and well-being, and to help them navigate service systems to achieve those goals.
 
DUTIES &amp; RESPONSIBILITIES:

<li>Conduct group presentations to engage and inform substance users about harm reduction interventions.</li>
<li>Conduct harm reduction-oriented individual and group education encounters with substance users to reduce the transmission of blood-borne diseases, other infections, and drug overdose.</li>
<li>Enroll individuals in State-authorized syringe exchange program and provide exchanges that promote the consistent use of sterile syringes and safe collection of used syringes and provide safer injection counseling.</li>
<li>Distribute Naloxone kits to reverse opioid overdose and provide group and individual training on their correct use of Naloxone.</li>
<li>Conduct HIV and HCV counseling, testing, partner notification and linkage services, in accordance with standards set by NYS Department of Health/AIDS Institute and CHASI.</li>
<li>Deliver community-based education and risk reduction education and materials to targeted populations and the community.</li>
<li>Provide information, referral, navigation, and coordination assistance that links individuals to health and human services responsive to their individually expressed needs and interests.</li>
<li>Collaborate with harm reduction and community access staff on the identification of service provision, trends in local drug use, and service providers that are substance user friendly.</li>
<li>Support the food pantry staff and volunteers during operations when needed.</li>
<li>Complete accurate documentation service provision using hard copy tools and electronic data systems.</li>
<li>Track inventory of harm reduction program supplies.</li>
<li>Complete internal and external training to maintain knowledge and skills related to harm reduction, culturally competent service delivery, and other job-relevant topics.</li>
<li>Actively participate in staff meetings and other agency activities.</li>
<li>Encourage a sense of commitment, camaraderie, and accountability among team members by modeling these qualities.</li>
<li>Screen incoming phone calls and provide administrative support on a periodic basis.</li>

Job Description:
As a key member of the referral services team, a Referral Specialist will serve as the primary contact to a referral source (prescriber, provider, healthcare center, system, and/or practice), the patient for whom the referral is for, and the field-based sales representative associated with the referral source account. In this role, the Referral Specialist will be responsible for navigating a new prescription via a referral from a referral source through the necessary steps to determine whether it can be serviced in the BioMatrix network of pharmacies.
A Referral Specialist will be aligned to a geographic territory of referral sources and corresponding field-based sales team member(s). This role will work closely with data entry, reimbursement, nurse coordination, and pharmacy operations team members supporting various stages of the referral and pharmacy management processes and will understand the referral status and next steps for each referral they are responsible for supporting. The Referral Specialist will build and maintain strong and lasting relationships with key personnel within the referral source environment using effective and timely communication ensuring their patient referrals are managed through service determination in a seamless manner.
QUALIFICATION REQUIREMENTS
