The Gary and Mary West PACE (Program of All-inclusive Care for the Elderly), affiliated with West Health (<a href="http://www.westhealth.org" target="_blank">www.westhealth.org</a>), is a non-profit that delivers medical, social, and behavioral services to the frail elderly to the North County of San Diego. This PACE center is entrepreneurial in nature and focuses on innovations and bringing new processes to the traditional PACE environment.

Position Summary:
Under the direction supervision of the PACE Clinical Services Director, the PACE Referral Specialist is responsible for coordinating and processing medical, dental, and optical referrals and authorizations for a variety of settings that PACE participants utilize. This requires the understanding of universal codes such as Current Procedural Terms (CPT), Healthcare Common procedure Coding System (HCPCS), and Durable Medical Equipment, Prosthetics, Orthotics, ad Supplies (DMEPOS) in addition to International Classification of Diseases (ICD-10). These codes will meet the requirements for specialty services to ensure minimal delay in securing accessibility for participants.
Essential Functions:
<ul>
<li>Receives and processes referrals and authorizations in a timely manner.</li>
<li>Coordinates care with primary care providers and their clinical coordinators in obtaining the necessary documentation needed for outsources care and diagnostics/radiology.</li>
<li>Interfaces with specialty offices to answer questions in regards to referral and authorization statuses.</li>
<li>Ability to navigate and utilize multiple functions from an electronic medical record platform in producing a comprehensive referral that will satisfy the needs of specialists.</li>
<li>Ability to use clinical and administrative judgement under regulations to determine best approach for both urgent and non-urgent patient care needs.</li>
<li>Maintain HIPAA standards and adhere to policies and procedures that govern the confidentiality of healthcare sensitive information.</li>
<li>Request additional medical records from specialty offices and clinic for further review by Utilization and Network Manager and/or PACE Medical Director as needed.</li>
<li>Maintains established departmental policies and procedures, objectives, quality assurance program, safety and environmental.</li>
<li>Enhances professional growth and development through participation in educational programs, current literature, in-service meetings and workshops.</li>
<li>Attends meetings as required and participates in committees as directed.</li>
<li>Ability to multi-task, be flexible, ensure accuracy, and meet changing priorities in a fast-paced, high-workload environment.</li>
<li>Ability to work with others as a core centralized team in addition to maintaining good interpersonal interactions with clients as a member of a multi-disciplinary community health team with a diverse multi-cultural population.</li>

Education and Certification:

Skills and Experience:
<ul>
<li>One year of health care experience working with frail and/or chronically ill elderly population.</li>
<li>One year experience in a healthcare setting including front and back-office exposure.</li>
<li>One year experience in referral and authorization processes for managed care insurance platforms.</li>
<li>Must have excellent written and verbal communication skills and demonstrate the ability to interact clearly and effectively with both internal and external customers.</li>
<li>Prior exposure working with and electronic medical record platform. Proficient in Microsoft Word and Excel.</li>
<li>Skilled in establishing and maintaining effective working relationships with participants, co-workers, vendors, medical staff, and the public.</li>

Referral Specialist

Job Summary
The Support Services Specialist will be responsible for responding to inquiries from community members, professionals, and internal staff across multiple communication platforms. They will professionally assess requests confidentially and provide accurate guidance, utilizing approved educational materials, referral resources, and database tools. This role involves maintaining and updating a comprehensive online database of national, state, and local resources to effectively meet community needs. In collaboration with the Associate Director of Support Services, you will support the creation and distribution of fact sheets and educational materials.
Additionally, the Support Services Specialist will implement and carry out initiatives for the Amputee Coalitions national support programs, helping to expand the reach and impact of Peer Support services, ensuring that individuals affected by limb loss and limb difference receive the knowledge and community support they need.
Essential Functions
<ul>
<li>Provide concise, accurate, and timely information in response to all phone, email, written correspondence, web-based submissions, and online forums from community members, family members, and professionals in the limb loss and limb difference community.</li>
</ul>
<ul>
<li>Support the national Certified Peer Visitor program by matching peer visit requests to the appropriate visitor. This includes case management of volunteers to evaluate the program's success.</li>
</ul>
<ul>
<li>Respond to all inquiries professionally and courteously.</li>
</ul>

<li>Enter all essential demographic information into the consumer inquiry database (Salesforce) to facilitate rapid, accurate, comprehensive statistical reporting and data transfer.</li>

<li>Effectively search various databases and websites to provide accurate and appropriate responses to inquiries.</li>

<li>Follow best-practice guidelines established by Inform USA for providing information and referral services.</li>

Job DescriptionDirect the comprehensive treatment of clients to achieve a healthier and more active lifestyle, providing services to develop, maintain, and restore maximum movement and functional ability throughout the lifespan. Responsible for the implementation of treatment, monitoring, documenting results, and determining appropriate termination of services, when possible. Specific exercises, manual therapy and manipulation, mechanical devices such as traction, education, physical agents including heat, cold, electricity, sound waves, radiation, and prescription of assistive devices, prostheses, and orthoses, as well as other interventions, may be included. Responsibilities: - Conduct screenings and contribute to the evaluation of clients, as appropriate, to determine the need for intervention/treatment. - Implements treatment plan developed by supervising physical therapist; implements, conducts, and monitors clients response to treatment. - Communicate with client, family, and team members about an appropriate home or community program to maintain or enhance the clients performance. - Provide/recommend/fabricate adaptive devices or other equipment, train client, family, and appropriate staff in the use or application of orthotics and prosthetics. - Adapt treatment and programs to meet the individual needs of the client. - Document results of clients assessment, treatment, follow-up, and termination of services. Participates in discharge planning. - Adheres to department procedures regarding documentation of Physical Therapy services. Records indirect time and billable treatment times by department procedures. Maintains the productivity level established by the Therapy Director. Complies with all Infection Control, Universal Precautions, and OSHA standards for the healthcare professional. Maintains a safe and clean work area. - Complies with Easter Seals policies and procedures, utilizing the highest standards of personal and professional ethics and behavior. - Demonstrates good judgment, diplomacy, courtesy, and effective communication while working at Easter Seals or when representing the organization. | Required Credentials: Current Florida Department of Health License (or license eligible).
