Performs required pre-certification, credit referral or deposit collection. Enters data in computer and thoroughly documents any incomplete admissions/registrations in manner prescribed. Obtains pre-certification, referral or authorization number and updates patient's file.

Notifies patients, family members, physicians and/or supervisors of insurance coverage issues, notifies patients of co-payments, deductibles or deposits needed, documenting all information in computer system.

 About the Opportunity
Pair Team is expanding its partnership with one of our largest health plan partners through a new field-based care management pilot focused on improving patient engagement and access to care.
The Field Care Coordinator plays a critical role in bringing care directly to our members where they are, both at home and in the community. This role focuses on hands-on, relationship-based support through in-person visits, new patient onboardings, and field-based outreach. Field Care Coordinators help members overcome barriers to care by assisting with everything from care plan follow-up and blood pressure checks to grocery or care package drop-offs.

This position is responsible for helping admit clients to the appropriate mental health program and facility by maintaining admitting guidelines; obtaining applicant information; screening applicants; completing admissions process; resolving patient dissatisfactions. The Admission Coordinator is also responsible for helping the client transition to the appropriate program while ensuring compliance with applicable standards. 
PRIMARY DUTIES/RESPONSIBILITIES:

This position requires strong interpersonal and organizational skills to build rapport with members and to coordinate referrals and care amongst various healthcare providers and community services. The ECM case manager also works with the member’s interdisciplinary team in supporting the member. The case manager engages member and member support systems to define priorities that are central to the member’s desired needs and goals. 
Requirements:
Education: Bachelor's degree in social work, sociology, psychology, AA in related field, Certified Nursing Assistant or bachelor's degree in related field (recreational therapy, human development, etc.) is preferred, OR 3 years of experience in a case management/care coordinator role.
