What You Will Do:
The Admissions Coordinator is responsible for managing inpatient and outpatient admissions. Registers incoming patients, enters information on admitting forms, explains hospital regulations, and assigns patients to rooms based on the nature of the illness and the type of accommodations available. May prepare identification bracelets and assist in insurance matters. Serves as a liaison between patients, their families, visitors, and the hospital (administration, management, hospital staff, medical staff, etc.)
Type: Full Time and Part Time, 100% Onsite
Schedule: Varied Schedules. Admissions Coordinators are required to work 1 shift every other weekend.
Location: Medical West Main ER, SDS, and Hoover ER

Responsibilities and Duties:

Responsible for obtaining complete and accurate demographic and financial information from a variety of sources, including patient interviews physician offices and in-house departments.

Obtains required signatures on legal consents and insurance forms.

Performs required pre-certification, credit referral or deposit collection. Enters data in computer and thoroughly documents any incomplete admissions/registrations in manner prescribed. Obtains pre-certification, referral or authorization number and updates patient's file.

POSITION SUMMARY

The Director of Admissions is responsible for the activities of Access Team members to respond to the referral request in a timely manner, ensures the referral and admission processes are completed in accordance with applicable regulations and hospice policies, oversees training of other team members in the admission processes and works with other departments and services to ensure the smooth transition of patients from the Access Team to the appropriate hospice interdisciplinary group (IDG). 

QUALIFICATIONS
 
Education: Graduation from an accredited school of nursing; BSN preferred.
Licensure: Current licensure as a Registered Nurse in Arkansas. 
Experience: A minimum of three years experience in a hospital, home health agency, hospice, or related program is required as well as a minimum of three years supervisory experience.
Additional Requirements: Valid Arkansas driver’s license, personal vehicle, and motor vehicle insurance. Must be able to travel to various locations.
 
PRIMARY RESPONSIBILITIES 

 1. Oversees intake/access staff and entry of all referral calls into the electronic medical record.
 2. Maintains a working knowledge of Medicare criteria for hospice eligibility and hospice levels of care and ensures education is provided to staff as needed.

<li>Tracks information weekly on the referral and admission process (e.g., timeliness, conversion rates, etc.) to identify opportunities for improvement. Meets established AH admission goals. </li>
<li>Ensures that the Access team tracks patients not admitted and ensures timely and appropriate follow-up. Meets established AH follow up goals.</li>
<li>Assists with making the hospice admission visit when needed. Will make at least one visit every 6 months to maintain skills.</li>
<li>In conjunction with Access Coordinator, provides monthly orientation and training class on referral and admission process.</li>
<li>Appropriately addresses any patient care issues or complaints according to Arkansas Hospice policy.</li>
<li>Collaborates with appropriate staff member when a new facility contract is needed for admission of potential patient to Arkansas Hospice to ensure timely response.</li>
<li>Coordinates staff activities to meet organizational objectives.</li>
<li>Maintains knowledge of all hospice services.</li>
<li>Assures that services provided are in compliance with state health department/federal government standards and regulations for licensure, and hospice certification.</li>
<li>Oversees the day to day functions, staffing and afterhours coverage of the UAMS team. This includes quality reporting and coordination with UAMS staff.</li>
<li>Ensures that documentation is completed daily in the patient’s electronic record.</li>
<li>Participates in Administrator On-Call rotation.</li>
<li>Assists in the development of policies, procedures, standards, and guidelines to fulfill the hospice program’s patient care objectives.</li>
<li>Evaluates staffing requirements; recruits, selects, hires, and participates in the assignment, orientation, career development, and performance evaluations of all Access.</li>
<li>Participates in program development and overall agency planning. Informs staff of plans, problems and progress.</li>

<li>Participates in community programs for the purpose of providing education and information about hospice in general and Arkansas Hospice. </li>
<li>Works with the Information Technology department, to assure effective, efficient, and secure use of clinical records and electronic communications.</li>
<li>Conducts monthly staff meetings with all direct reports; ensures that written meeting minutes are available for review by staff not in attendance, and attendance records and meeting agendas are maintained.</li>

4. Assess/Evaluate Resident care needs and assist in the development of individualized plans of Resident care as related to individualized Resident needs, family involvement and the physician’s plan of care for the Resident.
