JOB SUMMARY 
Under the direction and supervision of the Home Care Coordinator, RN the Home Health Aide is responsible for providing and assisting participants with activities of daily living and supportive care necessary to the preservation of the home environment. Assists in monitoring the health status and providing a safe environment for participants. Functions as a member of the multidisciplinary team. Promotes professional working relationships with both internal and external customers. Adheres to and supports all organizational policies and procedures and standards. Promotes teamwork! 
ESSENTIAL DUTIES AND RESPONSIBILITIES

DEPARTMENTAL (JOB SPECIFIC TASKS): 
<ul>
<li>Must maintain calm and professional demeanor. </li>
<li>Assists participants with activities of daily living (bathing, dressing, grooming, hair care, mouth care, eating, hand and foot nail care and toileting) as assigned or needed.</li>
<li>Assists in monitoring participant’s health status (vital signs, weight, skin condition, appetite and changes in functional ability). Observes participant’s taking prescribed medications. Reports pertinent information to Home Care Coordinator in a timely manner.</li>
<li>Assists participants with ADLs (grocery shopping, cleaning, changing bed linens, laundry, meal preparation and other routine household care) as assigned. </li>
<li>Documents services provided to participants consistently and accurately via Electronic Health Record (EHR). Follows procedure for flow sheet completion. Documentation is completed in a timely manner.</li>
<li>Participates in participant’s care planning through cooperation with multidisciplinary teams. </li>
<li>Participates in Quality Assurance programs as assigned</li>
<li>Handles potentially infectious specimens with appropriate biohazard precautions and practices universal precautions.</li>
<li>Ensures customer satisfaction. </li>
<li>Participates in Company required and assigned training programs and any required staff and training meetings. </li>
<li>Responsible for checking Company email at least once per week to receive updated scheduling information. </li>
<li>Maintains proper Boundaries when dealing with participants. Notifies supervisor of any possible ethical or boundary issues immediately upon discovery. </li>
<li>Responsible for maintaining professional affiliations and any required certifications.</li>
<li>Additional responsibilities will be added at managers discretion, as career growth opportunities and responsibilities are constantly arising in our flexible and start-up environment. </li>

QUALIFICATIONS 
<ul>
<li>High school diploma or equivalent.</li>
<li>Must be an active Certified Nurse Assistant (CNA) in Maryland. </li>
<li>Must have a driver's license and reliable transportation</li>
</ul>
<ul>
<li>Minimum of one year of experience with a frail or elderly population required.</li>
<li>Must have excellent written and verbal communication skills and demonstrate the ability to interact clearly and effectively with both internal and external customers. </li>
<li>Must submit to and pass a pre-employment drug/alcohol screening and criminal background check. </li>
<li>Must have a current Driver’s License and driving record must be in compliance with Company policy. Must maintain current automobile insurance. </li>
<li>Must be clear of all communicable diseases and have all immunizations up to date before providing direct care. </li>
<li>Must be efficient with computer applications such as email, navigating the internet, using electronic timekeeping, and Electronic Health Records. </li>
<li>Skilled in establishing and maintaining effective working relationships with participants, co-workers, vendors, medical staff, and the public. </li>
<li>Must be free of communicable diseases that can be readily transmitted in the workplace. TB test is required within the first 7 days of employment. HepB vaccinations are offered but not required. </li>
<li>Operates in full compliance with all applicable federal, state and local fire, health, safety, sanitation, Emergency procedures and other Standards prescribed in law or regulations.</li>
<li>Ability to work independently with minimum supervision. </li>
<li>Knowledge of how to use and maintain in-home equipment (adaptive, hoyerlift, etc.).</li>
