<li>Develops and leads group counseling and participant support group activities. </li>
<li>Refers participants and families to appropriate community agencies or facilities, serves as a liaison with such organizations and as an advocate for participants.</li>

<li>Assists participants with ADLs (grocery shopping, cleaning, changing bed linens, laundry, meal preparation and other routine household care) as assigned. </li>
<li>Documents services provided to participants consistently and accurately via Electronic Health Record (EHR). Follows procedure for flow sheet completion. Documentation is completed in a timely manner.</li>
<li>Participates in participant’s care planning through cooperation with multidisciplinary teams. </li>
<li>Participates in Quality Assurance programs as assigned</li>
<li>Handles potentially infectious specimens with appropriate biohazard precautions and practices universal precautions.</li>
<li>Ensures customer satisfaction. </li>
<li>Participates in Company required and assigned training programs and any required staff and training meetings. </li>
<li>Responsible for checking Company email at least once per week to receive updated scheduling information. </li>
