Position Overview 
The Charge Nurse oversees nursing staff during shifts, ensures patient care quality, manages supplies and household operations, and handles immediate clinical needs. Key Responsibilities

<li>Patient Care Quality Assurance

<li>Conduct quality assurance (Q/A) checks on patient charts for compliance and accuracy.</li>
<li>Verify patient schedules and authorizations regularly.</li>
<li>Act as the escalation point for complex patient concerns and issues.</li>

<li>Medication and Order Management
<ul>
<li>Add new medications to patient records as necessary.</li>
<li>Update and maintain NARC (Narcotic) sheets.</li>

<li>Use OmniView to refill medications and ensure accurate documentation.</li>
<li>Patient and Household Supplies Management
<ul>
<li>Order and inventory DME, respiratory, food, household, and personal supplies.</li>
<li>Verify inventory received against packing slips and resolve discrepancies.</li>
<li>Oversee household cleanliness standards, organizing daily room checks and general house operations.</li>

<li>Transportation and Patient Logistics
<ul>
<li>Request transportation for patients and ensure smooth coordination.</li>
<li>Order and manage specific food needs, such as Bridget’s food.</li>

<li>Clinical Care and Support

<li>Provide direct patient care as necessary, including administering medications, monitoring vitals, and assisting with treatments.</li>
<li>Serve as a clinical resource for nursing staff, addressing complex patient needs.</li>
<li>Perform APD Monthly Monitoring Visits and document findings.</li>

<li>Administrative and Communication Duties
<ul>
<li>Respond to voicemails, texts, and calls on the Charge Nurse cell phone.</li>
<li>Manage tasks in Microsoft Planner to ensure operational efficiency.</li>
<li>Regularly update and maintain Plan of Care and Standard Orders documentation.</li>
<li>Manage and distribute eFax files in Worldview.</li>
<li>Ensure the "Clip Board" is current with necessary information for staff reference.</li>

<li>Documentation and Reporting

<li>Complete incident reports and escalate critical incidents to the DON.</li>

<li>Ensure accurate documentation of patient care activities, assessments, and plans.</li>

Required Qualifications

Upper Room AIDS Ministry (URAM). URAM provides healthcare services from the Federally Qualified Health Center located in the heart of Harlem. We provide Primary Care, Specialty Care, Dental Care and Wellness Care to those seeking diagnosis and treatment. All employees of URAM are responsible for performing their job practicing good risk management, quality assurance, and delivering excellent internal and external customer service.
Reporting to the Medical Director, the Charge Nurse is responsible for planning, organizing, and directing the day-to-day clinical operations for primary care, specialty care (podiatry and Cardiology), and mobile health. This includes the scheduling, management, and oversight of patient flow. The Charge Nurse will work collaboratively with clinical directors of all departments, coordinators and the entire health service management team to ensure compliance with all regulatory/contractual guidelines for Article 28, 31 and FHQC-H. 
The Charge Nurse is as a team leader and is responsible for managing, coordinating, and educating multiple patient care team members including, but not limited to, Nursing Supervisor, Nurses, Medical Office Assistants (MOAs). Duties include, but are not limited to, delivering patient care, mentoring, evaluating, and training staff. In addition to the supervisory role, Charge Nurse overseas, infection control and central sterilization processing.
Essential Duties and Responsibilities 
Essential duties and responsibilities include, but are not limited to, the following:
Administrative

<li>Manages clinical services for all health services sites (The Nest, Willis Green, Mobile Units) to ensure the delivery of culturally competent, evidence-based, patient-centered care by all team members.</li>
<li>Provides oversight of clinical operations by providing direct supervision to, and coordination of, Nurses, Medical Office Assistants (MOAs).</li>
<li>Works with the management team to plan, organize, and direct all clinical services within each department.</li>
<li>Ensures adequate RN and MOA staffing</li>
<li>Defines the philosophy, purpose, policies and procedures, and standards for RN, LPN, Medical Offices Assistants, and dental assistants in collaboration with department directors.</li>
<li>Ensures RNs and MOA clinical competencies evaluations are completely in a timely manner ( 90 days for new staff, annually for all. competency. </li>
<li>Generates and implements both an onboarding and a continuing education curriculum for RN’s, MOA’s aimed at maximizing staff retention and professional development.

<li>Fosters an environment in which each RN and MOA practices to the top of their license, certification, and/or skillset and role. </li>
<li>Ensure appropriate training, error monitoring and correction are completed in a timely manner.</li>

Clinical Work
<ul>
<li>The nurse is expected to be proficient with all the tasks performed by the Medical Office Assistants, License Practical Nurses and Registered Nurses (see separate job descriptions). </li>
<li>The Charge Nurse is expected to provide nursing care to patients and perform the duties of the team they supervise based on staffing needs in order support clinic operations and ensure appropriate patient flow. </li>
</ul>
Quality Assurance
<ul>
<li>Revises and maintains the organization’s infection control and occupational health policies.</li>
<li>Enforces adequate Infection Control standards, with guidance from the Medical Director</li>
<li>Oversees the nursing team responsible for:

<li>Ensuring all clinical equipment is working properly.</li>
<li>Medications, Vaccines and Clinical Supplies Inventory</li>
<li>Monitoring Clinical logs</li>
<li>Environmental rounds</li>

<li>Oversees the Vaccine for Children, Vaccine for Adults and other vaccine programs.</li>
<li>Acts as point of contact and coordinator for the New York Citywide Immunization Registry (CIR)</li>
<li>Ensure annual OSHA training is completed and Conducts rounds to ensure compliance with OSHA safety standards.</li>
<li>Works in Collaboration with the Quality Improvement and Population Health team</li>
<li>Collaborates with other Management Team members as well as other clinical leaders within the organization on issues of patient care, clinical quality, compliance, customer service, utilization management, care management, patient access, system redesign, safety, and health center strategy. </li>
<li>Participates in the organization’s readiness programs for regulatory surveys, including but not limited PCHM, FTCA, and HRSA, DOHMH, AIDS Institute site visits. </li>
<li>Works in Collaboration with Human Resources, Medical Director and Senior Director of Special Projects in assisting in the implementation/maintenance of NYSDOH regulations such as but not limited to: Influenza and other vaccinations, Influenza Mask Protocol, Fit testing and other policies related to infectious diseases. </li>

Miscellaneous

<li>Schedule: Four days from 8:30 to 4:30 and one day 10:00- 6:00 PM</li>

Charge Nurse Charge Nurse LPN Qualifications:

Education: Associate’s or Bachelor’s Degree in Nursing from an accredited institution.

Licensure: Valid Registered Nurse (LPN/RN) license.

Experience: Minimum of 2 years of nursing experience, with at least 2 years in a skilled nursing or long-term care setting preferred. Previous supervisory experience is a plus.

Skills: Strong leadership, organizational, and communication skills. Ability to make sound clinical decisions and manage multiple priorities effectively.
</li>
<li>

Position Purpose
Provides direct nursing care to the residents and supervises the day-to-day nursing activities performed by the certified nursing assistants in accordance with current federal, state, and local regulations and guidelines and established facility policies and procedures. 
Required Qualifications

<li>A Nursing Degree from an accredited college or university or a graduate of an approved LPN/LVN program.</li>
<li>Prior experience in long-term care preferred but not required.</li>
<li>Current unrestricted license as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in practicing state.</li>
<li>Current CPR certification.</li>

Major Duties and Responsibilities
Directs the daily activities of the certified nursing assistants in accordance with current federal, state and local regulations and guidelines and established facility policies and procedures. 
Ensures that policies and procedures are complied with by nursing personnel assigned. 
Participates in the admission, discharge and transfers of residents as required.
Observes for changes in residents’ status, notifying the physician and resident’s family or representative and documenting accordingly.
Transcribes physician orders to medical record and carries out orders as written.
Prepares and administers medications as per physicians’ orders and observes for adverse effects.
Performs wound treatments as per physicians’ orders, observes for changes and documents accordingly.
Schedules follow up appointments for residents and transportation needs as indicated.
Provides nursing leadership to nursing personnel assigned to the unit.
Provides direct care skills such as colostomy changes, tube feedings, wound care, suctioning, IV administration, etc. in accordance with current policies and procedures. 
Performs rounds to ensure resident needs are being met and personnel are performing their assigned duties. 
Collaborates with other members of the interdisciplinary team as needed to ensure residents’ needs are holistically met. 
Informs appropriate departments of concerns or complaints and files grievances as necessary.
Reports any incidents or unusual occurrences to the supervisor, unit manager, assistant director or nursing or director of nursing and participates in the investigative process as needed.
Participates in end of life care as required.
Ensures that there is adequate stock of medications, supplies, equipment and notifies appropriate personnel of needs.
Initiates, reviews and updates care plans as required.
Promotes teamwork, mutual respect, and effective communication.
Participates in the survey process when inspection surveys arise.
