<li>Assist with wheelchair use, including pushing, folding/unfolding, and loading into autos.</li>
<li>Support Supervisors in creating menus, shopping, and safe food storage for client.</li>
<li>Prepare, provide instruction and/or assist in preparing nutritious meal that reflect individual preferences.</li>
<li>Will follow the direction of the Supervisor to implement the program plan.</li>
<li>Review documentation and discuss items with staff.</li>
<li>Will assist in keeping accurate data on the residents’ progress towards their individual goals.</li>
<li>Engage the residents on all tasks, training and community integration activities.</li>
<li>Report all incidents and occurrences to Supervisor including all incidents of possible abuse or neglect.</li>
<li>Report to Supervisor any resident’s needs that implies to be requested to the agencies.</li>

<li>Follow established emergency plan and procedures.</li>
<li>Will document all occurrences and observations on their proper forms every shift.</li>

<li>Resolves routine problems that arise and contact Supervisor if serious problems arise (behavioral crisis, medical crisis, etc.)</li>

MINIMUM QUALIFICATIONS
<ul>
<li>Prior experience supporting people with wheelchairs and Hoyer lifts.</li>
<li>To perform this job successfully, an individual must be able to perform each essential duty assigned, satisfactorily.</li>

<li>Will always present yourself appropriately, and as a role model for clients and other Direct Care Staff at home and in the community. Follow dress code.</li>
<li>Will attend any meeting and training per Supervisor request.</li>
<li>Will dispense or supervise dispensing of medication to clients.</li>
<li>Support Supervisors by coordinating staff for medical appointments and activities.</li>
<li>Assist Supervisors in the creation of the Activity Calendar with the staff and clients’ input.</li>
<li>Follow activities scheduled, keep residents active and always give them a choice.</li>
<li>Provide transportation to clients.</li>
<li>Follow established formal behavior support plans and guidelines for positive behavior support programs as outlined.</li>
<li>Assist in developing positive behavior supports by providing feedback about clients’ preferences.</li>
<li>Support client’s participation in cultural, recreational, and educational activities both at home and in the community.</li>
<li>Follow established emergency plan and procedures.</li>
<li>Will document all occurrences and observations on their proper forms every shift.</li>
<li>Provides leadership by displaying positive conduct and demonstrates initiative to enhance program.</li>
<li>Resolves routine problems that arise and contact Supervisor if serious problems arise (behavioral crisis, medical crisis, etc.)</li>
<li>Ensures proper maintenance and care of existing supplies, furnishings and equipment.</li>
<li>Receive receipts and documents for client finances.</li>
<li>Maintains clients’ confidentiality whenever appropriate. Refers sensitive communications to FOS. Maintains Confidentiality Regs.</li>

MINIMUM QUALIFICATIONS
<ul>
<li>Prior experience supporting people with wheelchairs and Hoyer lifts. </li>
<li>To perform this job successfully, an individual must be able to perform each essential duty assigned, satisfactorily.</li>

 Responsibilities Include but not limited to:                Provides support to program participants that meets their needs, provides opportunities forcommunity integration, including swimming, and creates a nurturing, safe, and healthful living environment. • Must be familiar with the Residential Service Guidelines issued by the Division of Developmental Disabilities.• Provides each participant with an array of instruction and support services, as described in the participants’ Individual Instruction and Support Plan (IISP).• Ensures that the daily needs of the participants are met, including personal hygiene, food preparation, house cleaning, shopping, and monitoring of health needs, including supervision of medication.• Assists participants with regularly accessing the community to take part in a variety of outings of the participants’ choosing. This may require transporting participants or arranging for transportation.• Ensures that participants are treated with dignity and consideration at all times. Promotes and safeguards each participant’s ability to make decisions and to freely exercise his or her rights.• Interacts with participants, provides informal counseling and problem-solves with participants.• Attends staff meetings as required by the Program Supervisor or the Site Supervisor and/or Administrator.• Attends training offered/required by the agency, DDD, and other parties.• Takes part in the development and implementation of the IISP for each participant with whom they work.• Will become Nurse Assistant Registered (NAR) at employee’s own expense initially and annually.• Must enroll and complete Nurse Delegation study course and take test within 30 days of employment.• Provides necessary support and supervision of participants.• Maintains professional working relationships with other services providers, the participants’ parents/relatives/guardians and other agency staff. Responsible for timely completion of all documentation, including, but not limited to, the medication log, shift notes, progress notes, data sheets, incident reports, and client financial records.• Performs other duties as assigned by the Program Supervisor or the Site Supervisor and/or the Administrator.  Requirements &amp; Qualifications:

<li>High School Diploma / Equivalent required</li>
<li>Washington State Driver’s License, a Reliable Vehicle, Auto Insurance, and must be willing to transport clients</li>

<li>Must be willing to undergo a background check, in accordance with local law/regulations</li>
<li>First Aid/CPR Certified (within 120 days of employment, training can be provided)</li>
<li>Have an active NAR/HCA/CNA (within 120 days of employment)</li>
<li>Blood Borne Pathogen (within 120 days of employment, training can be provided)</li>
