POSITION/TITLE: Nurse Practitioner
SUPERVISOR(S): Executive Director and Medical Director
STATUS: Full-Time - 32 hours weekly
POSITION SUMMARY: Bradley Free Clinic is seeking a compassionate, experienced, and mission-driven Nurse Practitioner to provide comprehensive primary care services to uninsured and underserved adults. As a key member of our interdisciplinary healthcare team, the Nurse Practitioner delivers evidence-based, patient-centered care while working collaboratively with physicians, specialist, behavioral health providers, pharmacists, dentists, nurse, clinical care managers, and a large network of volunteer healthcare professionals.
DUTIES:
DIRECT PATIENT CARE AND CONTINUITY OF CARE
- Provide comprehensive primary care services, including assessment, diagnosis, treatment, and management of acute and chronic conditions.
- Develop individualized treatment plans focused on prevention, chronic disease management, and improved health outcomes.
- Confer with in-house pharmacist and volunteer providers as neededespecially on patient treatment plans, goals and outcomes; lab orders and results; medication management; and other patient care needs.
- Perform complete and focused medical histories and physical exams as
- Order appropriate, cost-effective, and evidence-based laboratory and diagnostic tests and interpret findings.
- Prescribe medications as consistent with BFC protocols, standards of care, and state regulations.
- Review charts of patients requesting medication refills and issue prescriptions as
- Educate patients regarding condition, treatment, and possible
- Maintain accurate and timely medical charting with appropriate coding utilizing our Electronic Health Record system.
- Act as an advocate in directing patients to available clinic and community

ADMINISTRATION AND COORDINATION
- Daily priorities must include closing charts, review labs, and signing refill orders. Lab results and refill orders must be monitored for all patients.
- Work directly with medical department staff to oversee patient referral process, including timeliness of referral, patient communication, and follow up with ordering provider.
- Participate in chart review process with Medical Director(s).
- Report directly to Medical Director and Executive Director to keep informed on significant patent, clinical issues, and administrative issues on a daily basis.
- Participate in staff meeting, clinical committees, and organizational initiatives.
QUALITY IMPROVEMENT
- Participate in quality improvement activities as directed by the Medical Director.

QUALIFICATIONS INCLUDE:
- Current licensure as a Nurse Practitioner in the Commonwealth of Virginia.
- Current DEA registration.
- Current BLS certification.
- Minimum of 5 years of primary care clinical experience preferred.
- Strong assessment, diagnostic, and clinical decision-making skills.
- Excellent communication and interpersonal skills.
- Ability to work independently while collaborating effectively within a multi-disciplinary team.
BENEFITS:
Medical Insurance: Participating employees contribute 25% of the employee medical insurance premium while Bradley Free contributes the remaining 75%. Employees are responsible for 100% of dependents medical insurance premium. Employees who work 30 hours regularly will be eligible for medical on the first of the month following 60 days.
Dental Insurance: Participating employees contribute 25% of the dental insurance premium while Bradley Free contributes the remaining 75%. Employees are responsible for 100% of dependents dental insurance premiums.

Vision Insurance: Participating employees are responsible for 100% of the insurance premium. Employees who work 30 hours regularly will be eligible for vision on the first of the month following 60 days.

401K: Bradley Free Clinic will match employees contributions 100% up to 3% of the employees annual salary and 50% on the next 2% of the employees annual salary. Employees are eligible to participate once they attain age 21 and have 1 year service with the company. You will enter the plan on the first day of the month on or after you meet the eligibility requirements. Our 401(k) plan has a three-year cliff vesting schedule. This means you are always 100% vested in contributions you choose to defer; however, you must remain employed for three (3) years to be 100% vested in any employer matching contributions; there is no step vesting schedule.
