Summary:

Job DescriptionThe Center for Alcohol &amp; Drug Treatment is looking for a part-time (0.4 FTE) certified Nurse Practitioner or Physician Assistant to work in our Office-Based Opioid Treatment+ (OBOT+) program. The individual in this role would work in OBOT within HDC's Urgent Care Behavioral Health Care Center. You would see patients for a variety of needs related to substance use disorders, and may coordinate care between OBOT+ and other programs. You'll work with an on-site LPN and Patient Access Representative.&nbsp; Summary: The Nurse Practitioner is a key member of the CADT Medical Team and will be responsible to provide medication-assisted treatment for eligible patients. Essential Duties and Responsibilities:

<li>Understand emerging models of healthcare delivery that impacts CADT and its patients;</li>
<li>Identify and define new and innovative strategies to achieve business goals and objectives;</li>
<li>Identify opportunities to collaborate and develop clinical integration with other healthcare delivery systems to achieve positive and affordable outcomes;</li>
<li>Assist with various accreditation and licensing processes critical to CADT;</li>
<li>Actively engage stakeholders in the community to both receive feedback, represent CADT interests and services;</li>

Qualifications:
<ul>
<li>A current, unrestricted license to practice as an advanced practice nurse in the state of Minnesota;</li>
<li>A minimum of five (5) years’ experience in direct patient care;</li>
<li>At least one (1) year of experience providing Medication Assisted Treatment or prior work in a substance use clinic or hospital setting;</li>
<li>Holds a DATA waiver for providing Suboxone or is willing to seek immediate certification;</li>
<li>Knowledge of managed healthcare systems, medical quality assurance, quality improvement, and risk management;</li>
<li>Strategic and innovative thinker with the ability to communicate a vision and drive results;</li>
<li>Ability to problem-solve and work collaboratively across disciplines;</li>
<li>Experience in case management, disease management, and&nbsp;population health; and</li>
<li>Strong written and verbal communication skills;</li>
</ul>
Preferred Qualification:
<ul>
<li>Experience in behavioral health, substance recovery, or community health preferred;</li>
<li>Experience with diverse populations in an urban setting preferred; and</li>
<li>Preferred credentialed with insurance companies.</li>
