Job Summary
 
The MDS / Wound Care / Infection Control Nurse will assess and monitor patient health and well-being in accordance with federal guidelines. This position specializes in developing treatment plans and evaluating the effectiveness of Countryside Care &amp; Rehab’s patient services, as well as oversight of all infection prevention within the organization.
 
Duties and Responsibilities
<ul>
<li>Conduct patient assessments and develop patient care plans.</li>
<li>Make patient and facility recommendations in regards to MDS.</li>
<li>Oversight of Medicare and Medicaid programs.</li>
<li>Provide additional nursing support and coverage on the floor as needed.</li>
<li>Responsible for policies and procedures within the organization related to infection control.</li>
<li>Responsible for coordinating the infection control program which includes surveillance; analysis of data; developing reports, policies and procedures. </li>
<li>Use epidemiological principles and statistical methods to design, implement and evaluate infection prevention and control strategies. </li>
<li>Notify the County and/or State Health Department of any reportable diseases, adhering to all internal county and state procedures. </li>
<li>Actively engage in quality improvement activities.</li>
<li>Perform other duties as assigned or requested.</li>

Requirements:
Minimum Requirements

<li>Current nursing license (RN) for the state of Idaho.</li>
<li>Current Basic Life Support (BLS) certification.</li>
<li>Obtain MDS certification within 1 year if not already certified. </li>

</ul>
