POSITION: Registered Behavior Technician
REPORTS TO: Operations Manager and BCBA
Job Summary: The Behavior Technician will apply empirically based behavioral principles to all clients that the Behavior Technician works with. The Behavior Technician functions as an integral part of the treatment team and ensures that all necessary documentation is completed in a timely manner in order to benefits the clients that the Behavior Technician works with. This is an hourly position.
 Responsibilities/essential functions: The person in this position must be able to perform the following essential job functions with or without reasonable accommodation.
 
1. Applies empirically based behavioral principles consistently in all interactions with clients and staff
2. Implements Individualized Treatment Plans (ITPs) under the direction of the behavior analysts/consultants
3. Teaches the clients to acquire language, academic, self-help, and social skills as stated in the ITPs
4. Assists clients, as needed, to acquire academic and/or appropriate life skills. This may include toilet training, eating, and other self-help skills
5. Is a team-player and assists with ideas, suggestions and work duties
6. Actively participates in staff training programs designed to increase overall skills
7. Communicates in a professional and respectful manner with parents, behavior analysts/consultants, therapist trainers, therapists and clients
8. Becomes fluent in the understanding and administering of the VB-MAPP, ABLLS, and other assessments as needed
9. Observes records, and reports the condition and behavior of the clients, including maintaining daily records during the day
10. Works with clients, staff, families, and community to implement client's treatment plans
11. Consistently implements behavior plans to reduce client's socially inappropriate behavior and promote appropriate social behaviors
12. Effectively responds to and manages crisis situations which may include client physical aggression and property destruction
13. Completes daily paperwork and data collection notes in a detailed, accurate, consistent in a timely manner
14. Follows Adapt for Life and Adaptive Community Support Services policies and procedures, including adhering to confidentiality and HIPAA regulations
15. Submit all notes, progress reports, billing and other necessary documentation on time

Social Worker
JOB SUMMARY: The Social Worker (MSW) provides direct social work and care management services to participants of the PACE program, offering comprehensive support to enhance their quality of life and ability to live independently in the community. Responsibilities include participant screening, case management, counseling, and referrals, as well as planning, organizing, and implementing social work services tailored to participants and their families. 
As a liaison between the interdisciplinary team, caregivers, and community agencies, the Social Worker ensures participant needs are effectively addressed, and services are coordinated seamlessly. This role is essential in promoting participant well-being, advocating for their needs, and advancing the mission and goals of the PACE program. 
ESSENTIAL DUTIES AND RESPONSIBILITIES:

<li>Participate in initial assessments, care planning, and periodic reassessments (at least every six months) of participant cases, ensuring assessments are completed prior to team meetings and adhere to departmental standards.</li>
<li>Document participant changes and updates in the medical record accurately and in compliance with industry and departmental standards.</li>
<li>Collaborate and communicate effectively with Interdisciplinary Team (IDT) members regarding participant status, needs, and care plans.</li>
<li>Maintain up-to-date case management records, including periodic participant reassessments, and ensure comprehensive documentation.</li>
<li>Coordinate 24-hour care delivery and provide individual and family counseling to support participants and caregivers.</li>
<li>Develop and facilitate group counseling sessions and activities to address participant needs.</li>
<li>Refer participants and families to appropriate community resources, acting as a liaison and advocate to ensure access to needed services.</li>
<li>Foster effective relationships among team members, promoting team building and cohesion.</li>
<li>Consult with staff on the impact of social, emotional, and cultural factors on participant health, and advise on available social services in the community.</li>
<li>Participate in program and policy development for social work services and contribute to Quality Improvement (QI) processes.</li>
<li>Conduct home visits to participants’ private residences to evaluate and support their needs.</li>

EXPERIENCE AND EDUCATION:

<li>Master’s degree in Social Work from an accredited school of social work. </li>

<li>Current License in the required state. </li>
<li>Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home, or community-based setting is preferable. </li>
<li>1 year of experience working with a frail or elderly population preferred. If this is not present, training will be provided upon hiring. </li>

Provides day-to-day monitoring of equipment, lab tests, and maintenance activities. Performs these functions at all Water, Wastewater and associated Facilities. Holds after hours on call duties and works in after-hour emergency situations as needed.
Primary Duties/Responsibilities:&nbsp;
<ul>
<li>Operates equipment manually and through instrument panels, computer terminals, and programmable logic control units in an assigned area.</li>
<li>Performs field tests (chemical, physical, and biological) in order to pace chemical dosages for process and odor control units or alter unit processes.</li>
<li>Monitors and observes operating conditions, records observations and data in area shift log, and diagnoses operational problems and takes corrective action.</li>
<li>Follows process control procedures for assigned areas, verifies and reports suspected malfunctions and variances to supervisor.</li>
<li>Prepares chemical solutions, and monitors process chemical supplies and recommends reorder.</li>
<li>Samples wastewater, water, biosolids, and gases according to schedule and services automatic samplers and flow meters.</li>
<li>Performs process lab tests, and routine instrument calibration.</li>
<li>Performs routine and preventative maintenance, lubrication and minor adjustments on machines and related equipment which may include pumps, engines, centrifuges, generators, valves, gates, mixers, conveyors, blowers, dryers, chemical feed and odor control, disinfection equipment, vacuum filters and belt presses. Promptly reports any operational problems with machines or equipment to Leadman/Supervisor.</li>
<li>Learns to inspect and becomes proficient at mechanical troubleshooting to determine repairs needed. Dismantles equipment to gain access to and repairs defective parts using hand tools, power tools and precision measuring and testing devices. Re-assembles and tests equipment ensuring safe and efficient operating conditions is restored. Learns to troubleshoot hydraulic, pneumatic and electronic equipment and instrumentation</li>
<li>Must have the ability to operate computer maintenance management system (software) &nbsp;</li>
<li>Operates vehicles.</li>
<li>Assures work is performed in a safe and professional manner by following established safety, operating, and emergency response procedures.</li>
<li>Performs cleanup and housekeeping tasks for work area, tools, equipment, facilities, and grounds.</li>
<li>Performs the duties of Plant Operator and related duties as needed.</li>
</ul>
Work Environment:
<ul>
<li>Requires working in outside and inside environments with noise and all temperatures and climatic conditions, must be able to work under adverse weather conditions for extended periods.</li>
</ul>
Qualifications 
Education/Experience/Background:&nbsp;

<li>High School Diploma/GED is required.</li>
</ul>
Knowledge/Skills/Abilities:&nbsp;
<ul>
<li>Requires strong reading, math, writing, scientific, and verbal communication skills.</li>
<li>A mechanical aptitude.</li>
<li>Ability to read, interpret, and record data from meters, gauges, scales, panels, computer consoles, and other equipment.</li>
<li>Must demonstrate a working ability to use computer programs for process control.</li>
<li>Must have the ability to diagnose process problems in single or multiple unit processes and take remedial action; and to troubleshoot basic equipment.</li>
<li>Requires the ability to carry out verbal and written instructions.</li>

Required Certification/Licenses/Training:&nbsp;

<li>Requires obtaining a Wastewater Il within 24 months of appointment and a Wastewater Ill when all Indiana State requirements are completed.</li>
<li>Requires a WT2 in the State of Indiana within 24 months of appointment and a WT3 when all Indiana State requirements are completed.</li>
<li>Requires a DSL in the State of Indiana when all Indiana State requirements at completed.</li>
<li>Must have a valid driver's license and a safe driving record.</li>
</ul>
Physical Requirements:
<ul>
<li>Ability to perform tasks standing, walking, climbing stairs, squatting, lifting and while hands are extended above head or in front of body.</li>
