MDS Coordinator

Job Summary:

As a key member of our healthcare team, you'll play a crucial role in ensuring that our residents receive the highest level of care and attention. Responsibilities:

  • Demonstrate Envive's Values to our residents, family members, customers, and staff

  • Coordinate and manage the Resident Assessment Instrument (RAI) process, including assessments, care plans, and minimum data set (MDS) reporting

  • Collaborate with the interdisciplinary team to develop and implement comprehensive care plans that meet the unique needs of each resident

  • Ensure compliance with OBRA regulations, current state reimbursement guidelines, and other relevant laws and regulations

  • Maintain accurate and detailed records of resident assessments, care plans, and MDS data

  • Participate in quality improvement initiatives to ensure continuous improvement in resident care and services

Requirements:

  • Graduate of an accredited School of Nursing and hold a current nursing License

  • Minimum of one year in nursing management in the long-term care industry

  • Two years of professional nursing experience in long-term care, acute care, restorative care, or geriatric nursing setting

  • Knowledge of the Resident Assessment Instrument, Interdisciplinary Care planning process, and Minimum Data set 2.0

  • Knowledge of OBRA regulations and familiarity of current state reimbursement guidelines

  • Knowledge of current laws and regulations that influence provision of care and services in nursing facilities
