Housing Stability Coordinator

Job Description Summary:
The Housing Stability Coordinator is responsible for providing strength based, housing case management while engaging in compassionate and empowering problem-solving conversations with program participants, community partners, and team members to encourage housing stability. The coordinator will actively participate in the coordinated entry system as well as the larger service network as needed. The coordinator will also contribute to outcomes and continuous quality improvement at the agency and Continuum of Care level. 
Essential Functions:
<ul>
<li>Meet program participants to complete required paperwork and conduct a full assessment for the applicable grant funding and resources. </li>
<li>Provide housing case management services following Critical Time Intervention and Housing First models using a strength-based, person-centered approach. </li>
<li>Develop individualized housing stability plans in collaboration with the participant receiving services. </li>
<li>Explore long term affordable housing opportunities for program participants. </li>
<li>Conduct monthly home visits with housed program participants to review housing goals, continued eligibility, and exit planning. </li>
<li>Document all case notes within the Homeless Management Information System per grant and agency requirements.</li>
<li>Explore, identify, and link program participants to community resources related to their unique household needs. </li>
<li>Participate in community-based activities when appropriate, representing Community Housing Network at community forums, workgroups, and events as requested. Some activities may include evening and weekend hours.</li>
<li>Continue to refine skills and stay abreast of best practices and program changes through additional training and community events. </li>
<li>Navigate participant general and emergency support as needed.</li>
</ul>
Qualifications and Skills:
<ul>
<li>Experience in case management or related field preferred.</li>
<li>Strong communication skills, both verbal and written, with experience interacting effectively with vulnerable populations (i.e., people experiencing homelessness, domestic violence, at-risk of homelessness, and those who have a mental or physical health disability).</li>
<li>Lived experience; or Bachelor’s degree in social work or human services related field; or two years of human service-related experience and/or training, or equivalent combination of education and services. </li>
<li>Demonstrated ability to work independently and collaboratively in a fast-paced environment.</li>
<li>Working knowledge of the Coordinated Entry System and other community resources and services related to housing and supports is essential.</li>
<li>Must possess proficient computer skills, and be well organized, proactive, and self-directed. Must also be attentive to details and multitask in a fast-paced environment. </li>
<li>Valid driver’s license and personal automobile coverage limits minimum: Bodily Injury $100,000 each person, $300,000 each occurrence Property Damage $100,000 each occurrence.</li>

Work Environment and Physical Demands:

The Early Childhood Mental Health Therapist is responsible for providing outpatient services to children 4 to 6 years old and their caregivers who meet criteria for the program. The therapist will provide a variety of services including developmental screening and assessment, developmental guidance, relationship focused child-parent therapy, play therapy, and linking and coordinating of community resources and services. 
Other responsibilities include:

Requirements
To qualify for this position, you will need the following: 
<ul>
<li>Candidates must have a Masters in Social Work, Psychology, or mental health related field from an accredited program and the corresponding State of Michigan Licensure (LLMSW, LMSW, TLLP, LP, LLPC, LPC). Candidates without a state license can be considered but must obtain state licensure (LLMSW, TLLP, LLPC, LMSW, LLP, and LPC) within a 60-day training period beginning on the date of hire, this period may be extended by up to an additional 60-days of training at the sole discretion of the employer. Failure to obtain licensure within the training period will result in the employee being unqualified to perform the job functions. Beyond the training period, all state licensure must continually remain active, in effect, and free from disciplinary action. </li>
<li>Candidates will meet criteria for Child Mental Health Professional credential within 120 days of hire. </li>
<li>Must have one year of supervised experience in the examination, evaluation, and treatment of very young children and their families. Candidates should be working toward a MI-AIMH Endorsement, which can be earned during the first two years of employment.</li>
<li>Additional requirements include: knowledge of child development, a non-judgmental approach that reflects cultural sensitivity to the unique and diverse experiences of families served, the ability to assess and intervene within complex family and community systems, and a willingness to be flexible, patient and creative. </li>
<li>Must demonstrate competencies in the following areas: observation of parent/child interaction, developmental screening and assessment, birth to 5 child development, diagnostic skills, treatment planning, knowledge of Infant Mental Health theory, play therapy, understanding of the parallel process in therapeutic work, and collaboration with internal and external providers.</li>
<li>Candidates must have a valid driver’s license and appropriate driving record. Hours are based on client availability between 8:30am-7:00pm. Evening hours required to accommodate client work schedules, totaling 37.5 hours/week.</li>

This position will help to support multiple Rapid Rehousing programs, including working with youth (ages 18-24) and those who are fleeing domestic violence. 
Key Responsibilities:

<li>Assist Case Planners with program intakes.</li>
