Unity Health Care has launched a Food Pharmacy Program, a collaborative effort with Capitol Area Food Bank, to combat food insecurity for senior patients aged 65 years of age and older. The Food Pharmacy will target seniors who have hypertension, diabetes and/or obesity at Unity’s East of the River health center site. Under the supervision of the Medical Director, the Nutritionist will provide patient nutrition education and consultation. Also, the nutritionist will determine and prioritize the nutritional risks of clients based on established priority factors. 
 MAJOR DUTIES

<li>Provides nutritional instructions to patients 65+ living with hypertension, diabetes and/or obesity.</li>
<li>Disseminates Unity-CAFB designed toolkit that promotes health education around healthy foods etc., </li>
<li>Provides nutrition-based patient health education on tailored food prescription to address the patient's medical and nutritional needs, taking into consideration food preferences and cultural background.</li>
<li>Develops nutritional care plans for designated high-risk individuals and provides follow-up instructions, as required, to assure that the health and nutritional needs of the clients are adequately addressed.</li>
<li>Coordinates nutritional care with other health care members in order to ensure comprehensive medical and social services for clients, including referrals for related services, such as Food Stamps, Medicaid benefits, as required.</li>

Key Responsibilities:
This position is designed to manage road construction, and bridge projects in and around the area. The ideal candidate should have multiple years of experience working in a superintendent role in a fast-paced environment. Handling multiple sub contractors and under ground experience is a plus! This Superintendent is responsible for the timely completion of heavy civil projects within an allotted budget. The position will be responsible for scheduling the project activities and directly supervise various foreman in the construction of all earthworks, roadwork, bridges, or other performed work.

<li>Supervise total construction effort to ensure project is constructed in accordance with design, budget, and schedule.</li>
<li>Interfacing with Client representatives and other contractors, etc</li>
<li>Report any extra work, back charges to subcontractors, or delays caused by owner, utility company, or others.</li>
<li>Perform other related duties as required of the position or as assigned by supervision</li>
</ul>
 Qualifications:
<ul>
<li>One (1) to four (4) years of experience in specified skill (depending on mechanic level).</li>
<li>Post-offer drug/alcohol screen and physical evaluation.</li>
<li>Applicant/employee must not pose a direct threat to the health or safety of such individual or others in the workplace.</li>
<li>Job site safety requirements as required by the client (clean shaven, long sleeves, steel toe boots, etc.).</li>
<li>Safety equipment as required by the client (hard hat, safety glasses, gloves, respirator, hearing protection, safety harness with lanyard, etc.)</li>

Physical Requirements:

<li>Must comply with company’s drug and alcohol abuse policy.</li>

<li>Stoop, kneel, crouch, crawl, or work at elevated heights as well as in confined spaces.</li>
<li>Able to carry 50 lbs. of tool and/or materials. It is recommended to get assistance to lift items over 50 lbs. Check with your project supervisor or project safety director.</li>
<li>Must be able to stand or walk for long periods of time.</li>
<li>Must be able to work extended hours on your feet on uneven surfaces (dirt, concrete, asphalt, mud, gravel, etc.)</li>
<li>Must be able to work while wearing a respirator and other personal protective equipment, when required.</li>
