Position Summary
The Connection Center Resource Manager position supports the provision of Coordinated Entry of social services in Colorado by providing information, referral, intake assessment, and emergency casework services over the phone, via email, video chat, and/or in person. The Connection Center Case Manager will provide case management services using homeless diversion and prevention strategies and will also be responsible for using HMIS and Salvation Army databases to document assistances, referrals, and to track assisted households monthly, for at least one year. Provides easy access to tailored solutions for families in Metro Denver that are experiencing homelessness, are at risk of experiencing homelessness, or who are precariously housed, to promote long term housing stability. The Connection Center is open M-F from 7:00 am to 9:00 pm, and weekends from 8:00a to 12:00pm. Please note that these hours be extended in cold weather activation.
Duties and Responsibilities
General

CLM provides the area's most comprehensive array of mental health services for children and their families. Our multi-disciplined treatment team works with individuals ages 15-26 who experience mental illness, serious emotional difficulties and first episode psychosis. The Young Adult Community Case Manager provides care planning, resource identification, benefit coordination, service monitoring, and community-based counseling support to assist clients with improving symptoms and behaviors that otherwise interfere with age-appropriate development, functioning and wellness goals. This is a full time, 35 or 40 hour per week position and is considered a non-exempt, in accordance with the Fair Labor Standards Act (FLSA).
 
Major Functional Areas and Description of Duties
<ul>
<li>Provide individual assessment to create care plans that specify presenting problems, objectives, interventions, and desired outcomes.</li>
<li>Assist clients with applying for and maintaining adequate medical coverage to support consistent access to whole health care and treatment.</li>
<li>Provide outreach, advocacy, coordination, resource identification, linkage and monitoring of internal and external supports and services.</li>
<li>Provide counseling and Functional Support Services (FSS) in the home, school or other community-based setting as deemed appropriate for the objectives, interventions and desired outcomes identified in the client’s treatment plan.</li>
<li>Provide Individual Therapeutic Behavioral Services (TBSI) to develop, apply, and reinforce coping strategies to reduce and/or ameliorate symptoms and behaviors that interfere with functioning and goal achievement.</li>
<li>Encourage engagement in and provide Family Training and Counseling (FTC) services to educate and promote how to best support the client’s resiliency and recovery toward treatment goals.</li>
<li>Provide Crisis Intervention (CI) when clients experience acute exacerbation of symptoms to ensure their safety within the home and/or community.</li>
<li>Provide timely documentation on each service delivered.</li>
<li>Work collaboratively with other treatment team members, natural supports, and external providers to ensure a wrap-around family systems approach is provided to the client.</li>
<li>Provide set case management contacts per month andsethours of functional support services per week (eligible for incentive pay upon exceeding productivity expectation).</li>
<li>Serve on appropriate committees, both internal and external, as determined by supervisor.</li>
<li>Function professionally and in a manner that protects the integrity, confidentiality and rights of all patients.</li>
<li>Execute all required services and documentation in accordance with agency policies and federal/state standards.</li>

Physical Demands and Working Conditions
Half of the work will be visiting clients in the community and also in their homes. The other half will be office based. The ability to actively engage with children and conduct interactive play within a therapeutic setting is required. Manual dexterity is needed to use computers. The ability to sit for extended periods of time is required. Reliable transportation, a clean driving record, and proof of auto insurance coverage is required.
