Position Overview: We are currently seeking a licensed and experienced Registered Nurse (RN) Case Manager to join our caring and dedicated team. This role is crucial to ensuring patients and their families receive high-quality, compassionate care tailored to their unique needs. <li>Conduct thorough assessments of patients' physical, emotional, and psychosocial needs. </li>
<li>Develop and implement individualized care plans in collaboration with the interdisciplinary team. </li>
<li>Provide skilled nursing care, including symptom and medication management. </li>
<li>Coordinate resources, referrals, and care services for patients and families. </li>
<li>Offer education and emotional support to patients and their loved ones regarding end-of-life care and planning. </li>
<li>Maintain accurate and timely clinical documentation in accordance with hospice policies, Medicare/Medicaid guidelines, and applicable regulations. </li>
<li>Ensure compliance with HIPAA, state/federal healthcare laws, and organizational procedures. </li>
<li>Supervise and provide direction to Licensed Vocational Nurses (LVNs) and Home Health Aides (HCAs) as needed. </li>
<li>Participate in interdisciplinary case conferences, quality assurance activities, and continuing education. </li>
<li>Participate in scheduled home visits and on-call rotation to ensure 24/7 patient support. </li>
</ul>
Qualifications: 
<ul>
<li>Graduate of an accredited school of nursing. </li>
<li>Current and valid RN license in the state of New Mexico. </li>
<li>At least 1 year of nursing experience; hospice or end-of-life care preferred. </li>
<li>Strong critical thinking, organizational, and assessment skills. </li>
<li>Compassionate and empathetic approach to patient care. </li>
<li>Excellent verbal and written communication skills. </li>
<li>Valid driver's license and current automobile liability insurance. </li>
<li>Reliable transportation for patient visits in home and community settings. </li>
