Summary
The Nurse Practitioner / Physician Assistant provides high quality, patient-focused health care services to all patients in collaboration with the other staff members to ensure that the individually planned needs and desires of patients are met, regardless of age or gender, on a continuing basis by performing the duties outlined below. The Nurse Practitioner will practice/perform the medical services as delegated or directed by the supervising physician as long as they are within the scope of practice for the RN/APRN. The NP will abide by the standards and laws governing the practice of nursing and advanced practice registered nursing as established by the designated State Board and specified State Professional and Occupational Standards. It is the obligation of each Nurse Practitioner to ensure that their supervisory requirements are adhered to, that the relationship of and access to the supervising physician is defined, and that a process for evaluation is established. It is the obligation of each Nurse Practitioner to keep current with required continuing education and licensure. 
 
Essential Duties and Responsibilities include the following.

<li>Maintains appropriate clinical standards. Follows the guidelines established by the Company policy and procedure manuals.</li>
<li>Examines patients for symptoms and/or physical information while ensuring the delivery of high quality, patient-focused health care all while recognizing the patient’s individual rights. </li>
<li>Aides the patients in their right to be informed and to participate in making decisions in all aspects of their medical care and treatment and initiates a patient education plan according to the individualized needs of the patient, including patient and family instruction. Ensures that all aspects of care have respect for personal values and beliefs, and is focused on the individual needs of each patient.</li>
<li>Elicits and reviews information about patient’s medical history.</li>
<li>Orders or executes various tests, analyses, results, reports and diagnostic images to provide information on patient’s condition in a timely manner to determine appropriate treatment and follow up. </li>
<li>Administers or prescribes treatments and medications appropriate for patient condition.</li>
<li>Promotes health by educational instruction as well as advising patients concerning diet, hygiene, and methods for prevention of disease.</li>
<li>Refers patients to medical specialist for consultant services when necessary for patient’s well-being.</li>
<li>Accurately, efficiently, and completely documents necessary data for patient records to facilitate completion of billing processes. </li>
<li>All entries into the medical record whether electronic or on paper will be legible, complete, dated, timed and signed with appropriate credentials. Documentation will be completed prior to discharge from the facility when possible but no later than 48 hours after discharge from the facticity. </li>
<li>Utilizes all equipment, supplies, facilities, and resources of the Company in a prudent and efficient manner.</li>
<li>Participates in department quality improvement activities, staff educational programs, attends departmental meetings, and demonstrates and maintains clinical competency in the delivery of patient care services in accordance with Company guidelines.</li>
<li>Participates in Peer Review and Chart Audit processes as designated by the Medical Director. </li>
<li>Maintains supervisory duties over staff as required.</li>
<li>Must maintain collaboration with the physician to deal with issues outside of their scope of practice.</li>
<li>Maintains competency in skills relevant to the scope of services provided in the facility. The scope of services in specific markets will require employees to complete relevant training and to obtain additional certifications or licensure. (DOT Certification, Occupational Medicine Certification, Dispensing Permits, Telemedicine permits, etc.)</li>
<li>Other duties as needed.</li>

Requirements:
Education and/or Experience Master’s Degree in Nursing 
 
Certificates, Licenses, Registrations State Registered Nursing License; State Advanced Practice Registered Nurse License; Collaboration Agreement; CDS Controlled Substance License as applicable per prescriptive authority; DEA Drug Enforcement Agency Registration Number as applicable per prescriptive authority; Basic Life Support Certification. Active medical board certification required per state requirements.
 
Other Qualifications Successful completion of Advanced Cardiac Life Support (ACLS) is preferred for full time NP. 
Language Skills Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
 
Reasoning Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
 
Computer Skills Ability to perform this job successfully, an individual should have basic knowledge of EMR systems, specifically DocuTap, Excel spreadsheet software and MS Word Processing software.

JOB SUMMARY
Under the general direction of the Executive Director of Nursing and Clinical Resource Lead, the Registered Nurse Staff will lead the nursing team on all hospital units, accept responsibility for and provide direct and indirect patient care; supervise and provide direction of subordinates in accordance with TJC, federal, state, and local guidelines, organizational and departmental policies and procedures. JOB QUALIFICATIONS:
Minimum Level of Education: Education level equivalent to graduation from an accredited school of registered nursing.

Licensure, Certification, Registration: Must have and maintain an unencumbered Registered Nurse License in the State of Georgia, must have and maintain certification in BLSCPR and ACLS.

The Lead Certified Ophthalmic Assistant position is an advanced-level position including a wide variety of clerical, technical, and patient related functions under minimal supervision. This position performs all of the duties of an ophthalmic assistant with greater independence, including clinic flow management, documentation of exam findings during patient preparation and visit, patient education, assistance with medical examinations, treatments, and procedures, ordering and performing tests, coordination of area work assignments, protocol review and updates. In addition to performing and overseeing such tasks, the Lead COA is also a mentor and role model for the Ophthalmic Assistants and functions as a head worker for support staff.

<li>Collect, document completely and accurately, patient exam information including medical history, chief complaint, appropriate vitals, demographic information, updated medications, allergies, and health maintenance system.</li>
<li>Screen patient and perform accurate and consistent documentation and measurement tasks in relation to: Visual acuity, manual lensometry, confrontational visual fields, stereo testing, evaluation of motility, papillary function tests, color plates, applanation tonometry, advanced refractometry, retinoscopy, angle assessment, dilation of pupils, contact lens evaluation, visual field testing, optic nerve scanning, pachymetry, corneal topography, A Scan and B Scan, ophthalmic imaging including fluorescein angiography, etc. </li>
<li>Assist provider with laser procedures and minor surgery maintaining the practice standards of sterile technique and infection control.</li>
<li>Provide patient home care instructions and counseling. Clarify pre-op and post-op instructions with patients.</li>
<li>As team head, facilitate daily operations and responsibilities of the “patient flow” within the team. </li>
<li>Act as the first level, “go to person” for the providers and heads the team regarding troubleshooting issues and as a patient services liaison to research issues surrounding complaints, registration, and then communicating findings to clinical manager. </li>
<li>Assist clinical manager with provider schedules, coordinating vacation requests and absences to ensure appropriate coverage. </li>
<li>Ensure smooth office operations which support teamwork, and generate a positive attitude between front and back-office staff. </li>
<li>Attend regular EHR training sessions and distribute new information to providers and staff through individual trains and communications. </li>

Requirements:
Four years clinical experience in a busy general Ophthalmology practice setting.
- COA, COT or COMT certification. 
- Excellent clinical skills coupled with the ability train clinical staff.
