<li>Responsible, in coordination with the clinical team and IDT, for utilization management including minimizing unnecessary or preventable utilization, safe and effective discharges and transitions between care settings, and utilization review and improvement efforts to meet participant care needs.</li>
<li>In conjunction with the Director of Clinical Services, acts as a liaison between program and contracted providers and facilities for the coordination of effective care and treatment of program participants.</li>
<li>Functions as an engaged member of the leadership team. In coordination and partnership with other members of the leadership team, assists in the development and implementation of organizational and strategic initiatives.</li>
<li>Oversees the SCC Clinical Team. Supports the Clinical Team in ensuring the participant plan of care is fully developed and accurately reflects participant needs and care and services provided in alignment with organizational policy. Ensures clinical team adheres to the plan of care.</li>
<li>Oversees and is responsible for the system/process for ensuring both compliance and effectiveness of orientation and ongoing competencies for all staff providing clinical services including RNs, LPNs, CNAs, PCAs, CPhTs, and QMAPs. Ensures that clinical staff work effectively within their scope of practice.</li>
<li>Actively participates and engages in the implementation of quality improvement initiatives. Actively participates as a member of the Quality Assurance Committee.</li>
