The Case Manager assesses clients’ needs, plans a course of action, links client to services and monitors progress of clients and residents in their program. The Case Manager ensures that clients receive appropriate information and referrals to obtain and maintain housing. 

DUTIES &amp; RESPONSIBILITIES:

<li>Conduct intake and assessment process to orient client to the program and develop individual service plan reflecting assessment needs and goals.</li>
<li>Meet weekly with client to assist with the achievement of individual service plan goals.</li>
<li>Coordinate services with Housing Specialist, other staff and external partners</li>
<li>Provide advocacy for individual clients around entitlements and services.</li>
<li>Provide crisis intervention, aggressive and supportive counseling to strengthen clients’ ability to make appropriate life decisions.</li>
<li>Document case management activities in accordance with agency and funding source guidelines and procedures.</li>
<li>Complete Housing applications (both written and computer generated) for clients based on their individual needs and functioning.</li>
<li>Achieve monthly placement quota, as assigned.</li>
<li>Facilitate workshops, groups and escort clients as applicable.</li>
<li>Attend staff meetings and trainings to enhance skills that are related to job function.</li>
<li>Must be able to work flexible, alternate schedules which may include evening and weekend shifts.</li>
<li>Perform other related duties.</li>

EXPERIENCE, EDUCATION AND SPECIAL REQUIREMENTS:
Bachelor Degree in Social Work or Human Services Field preferred. At least 1 year of case management experience working with the homeless, mentally ill, substance abuse and/or low income populations. Bilingual a plus. Strong relationship building skills required. Ability to work independently but know when to raise issues to supervisor. Excellent organizational and time management skills necessary. Ability to effectively multi-task and follow-up on a timely basis. Strong PC skills required including Word and Excel.

Medical Appointment Responsibilities
1. Provide staff (Direct Care) coverage in the event of a shortage.
2. Meet with the Registered Nurse weekly, and schedule consumer medical, dental, psychiatric, and all other clinical appointments, as needed.
3. Must accompany consumers on medical appointments in the community.
4. At all times on duty must ensure that the consumers are appropriately dressed and groomed, before leaving the residence.
5. Must ensure that all consumer medications are administered as prescribed, and Medication Administration Records are accurately documented.
6. Must ensure that bagged lunches and snacks brought into the community are appropriate to the consumer menus.
7. Comply with all other duties which may be assigned by the Manager or Director.
8. Must meet with the Dietician on a monthly basis, in order to assure consumer nutritional needs are met.
Direct Care Responsibilities
Responsible for physical plant upkeep through daily maintenance of checks
Ensure that consumer body weights are taken weekly, and data is maintained.
Ensure that resident’s daily goals are conducted and data is appropriately maintained.
Ensure that behavioral goals are implemented, and data is properly maintained.
Ensure that residence vehicles are maintained, and travel log documented.
