Care Coordinators are primarily community-based and may be able to perform some tasks from home. Care Coordinator Non-Licensed will work with individuals, parents/guardians, Boundless staff, community members, and other service and support providers via face-to-face engagement, telephone, video conferencing, and electronic communication. Day-to-day activities are varied based on the needs of the individuals and families/ caregivers on the caseload. The Care Coordinator Non-Licensed serves as a primary point of contact and liaison for all the vital support providers in a youth or family's life, including scheduling meetings, tracking, exchanging documentation, following up on needs and appointments, and reporting outcomes.

<li>Track and report when less than two providers of a needed service are available within a community to offer an individual or family. Participate in team and community meetings regarding necessary resource development specific to the communities served.</li>
<li>Facilitate the completion of necessary forms and documentation, including consent for release information with relevant stakeholders and child and family teams.</li>
<li>Establish and maintain positive relationships with the local system of care partners such as the children's services system, Title IV-E agencies, boards of developmental disabilities, local corrections and court systems, schools, family and children first councils, managed care organizations, providers of behavioral health services, and primary care providers.</li>
<li>Support individuals and families in accessing medically necessary services and supports.</li>
<li>Serve as a liaison between individuals, families, and care team members to positively promote collaboration.</li>
<li>Complete all documentation as required, including gathering and reporting of all health and outcomes data.</li>
<li>Manage phone calls and communication from individuals, families, guardians, and stakeholders, etc. Coordinate meetings and ensure information is exchanged consistently.</li>
<li>Respond to and be available for clients and families in crisis, as needed, including referencing the crisis plans and facilitating the appropriate linkage to de-escalate the crisis.</li>
<li>Meet engagement, documentation, and contract requirements as established by management.</li>
<li>Provide excellent customer service and communication coordination between team members, individuals, families/ caregivers, community members, and collaborating organizations.</li>

The Care Coordination Supervisor is primarily responsible for ensuring adherence to the high-fidelity wraparound model of care coordination, ensuring quality care for children and families/caregivers representing diverse socioeconomic, racial, ethnic, and cultural backgrounds. The supervisor has a thorough understanding of local community resources and are skilled at developing and maintaining working relationships with staff, clients and community partners. This position will be responsible for serving Region H (Jackson, Pike, Meigs, Gallia, Highland, Fayette, Pickaway, Ross, Hocking, Vinton, and Athens Counties).
The pay range for this position is $64,000- $74,957.37 annually based on experience. LSW/LPC is required for this role.
 Essential Functions:
<ul>
<li>Supervise and oversee activities of care coordination team members through various methods including but not limited to team meetings/supervision and regular communications.</li>
<li>Provide training for direct care coordinators in the home and community setting.</li>
<li>Provide routine training, support and assistance to staff through individual and group conferences, assistance in analyzing case problems, and in improving their skills.</li>
<li>Promote an ongoing culture of team member recognition and create an atmosphere of teamwork.</li>
<li>Complete and maintain high fidelity wraparound training credential.</li>
<li>Ensure completion of skill and competency-based training to supervise delivery of ICC and MCC. Review, provide feedback, and approve all required documentation – ensure completion of all required care plans, initial assessments and CANS, outcomes reports, etc.</li>
<li>Ensures coordination and adherence of on-call rotation</li>
<li>Participate in ongoing fidelity review and monitoring system focused on consistent application of system of care principles, adherence to OhioRISE ICC/MCC planning process and service components.</li>
<li>Collaborate with all relevant systems and key participants within systems pertaining to children or youth and family/caregiver being served to ensure the highest quality of services are being provided with focus on ensuring family voice and choice.</li>
<li>Ability to assess needs in crisis situations and inform response.</li>
<li>Respects confidential information. Ensures operations meet quality and compliance standards.</li>
<li>Assures documentation meets Medicaid standards.</li>
<li>Evaluates performance of staff members and recommends appropriate action to the manager.</li>
<li>Participate in the review and selection of new staff, including but not limited to completion of position requisitions, applicant reviews, and interviews.</li>
<li>All other duties as assigned.</li>
</ul>
Minimum Requirements:
<ul>
<li>Bachelor’s/master’s degree in social work, education, counseling, psychology, or another related field preferred.</li>
<li>LSW/LPC required.</li>
<li>Three years of experience providing coordinated supportive services/care coordination services.</li>
<li>Previous supervisory experience strongly preferred.</li>
<li>Experience working in the behavioral health field required, preferably experience working with multi-system youth, or individuals with autism and/or developmental disabilities.</li>
<li>Working knowledge of local, state, and federal regulations for services for behavioral health, developmental disabilities, and autism.</li>
<li>Knowledge of high-fidelity wraparound services preferred (training provided post-hire).</li>
<li>Excellent communication skills, both oral and written, demonstrating professionalism with children, youth, families/caregivers, colleagues, and partners.</li>
<li>Excellent organizational skills with the ability to stay focused</li>
<li>Ability to prioritize multiple tasks</li>
<li>Demonstrated clinical knowledge of children with co-occurring developmental disorder and complex behavioral health issues</li>
<li>Experience working within Electronic Health Record systems</li>
