Position Overview
The&nbsp;Community Health Navigator&nbsp;(CHN) oversees AHI’s Afghan Health Coalition, coordinating workshops and community discussions, managing program reporting, and building stronger connections between Afghan women, healthcare providers, and community systems. This role ensures maternal health navigation is delivered in a culturally and linguistically responsive, trauma-informed way that advances trust and equity in healthcare.
 
Key Responsibilities
 
Program Implementation &amp; Supervision

<li>Coordinate workshops on prenatal, postpartum, and overall maternal well-being.</li>
<li>Manage distribution of program supplies, incentives, and resources.</li>
<li>Oversee day-to-day operations, ensuring alignment with program deliverables.</li>

Healthcare Provider &amp; Stakeholder Engagement
<ul>
<li>Act as a liaison between clients, healthcare providers, and community partners.</li>

Client Navigation Support

Data Management &amp; Reporting

Community Engagement &amp; Advocacy

Qualifications
 
Required

<li>Associate’s or Bachelor’s degree in Public Health or Social Services.</li>
<li>At least 1–2 years of program or project coordination experience, ideally in health, maternal health, or social services.</li>

<li>Experience with data management systems and reporting for health or social programs.</li>

Overview 
 
As a home health Occupational Therapist, you will be responsible for conducting one-on-one patient care and patient/family teaching in the home setting. You will work directly with patients in their residence implementing therapeutic interventions to optimize their functional abilities per Plan of Care to promote safety and independence in the home setting. 
Requirements:
<ul>
<li>Graduate of an accredited occupational therapy educational program </li>
<li>Current Occupational Therapy / OT state license within the state of practice</li>
<li>Minimum of one (1) year of occupational therapy experience in an inpatient, outpatient or home healthcare setting; home health industry experience required </li>
<li>Current CPR certification</li>
<li>Excellent organizational, time-management, written and verbal communication skills</li>
<li>Reliable transportation, current driver’s license and automobile insurance </li>
