Responsibilities
- Assist patients with daily activities such as bathing, dressing, grooming, and mobility to promote independence and comfort.
- Monitor and record vital signs, including blood pressure, pulse, temperature, and respiration rates, accurately documenting all patient information in electronic health records (EHR) systems.
- Support medical procedures by preparing patients and assisting healthcare professionals with tasks such as catheter care, phlebotomy, and basic life support during emergencies.
- Provide specialized care for individuals with dementia, Alzheimer's disease, memory impairments, or behavioral health needs by implementing tailored approaches like memory care techniques.
- Maintain strict adherence to infection control protocols using aseptic techniques and proper use of personal protective equipment (PPE).
- Assist with patient mobility using equipment such as Hoyer lifts and ensure safe transfer procedures in compliance with safety standards.
- Collaborate with interdisciplinary teams in various settings including acute care hospitals, long-term care facilities, assisted living communities, and outpatient clinics to deliver comprehensive patient service.
Qualifications
- Valid certification as a Certified Nursing Assistant (CNA).
- Demonstrated experience working in diverse healthcare environments such as hospitals (including ICU, trauma centers, emergency medicine), nursing homes, assisted living facilities, or home care.
- Familiarity with EMR/EHR system Eldermark for accurate medical documentation.
- Knowledge of medical terminology, anatomy, physiology, and infection control practices essential for quality patient care.
- Experience with specialized procedures including medication administration, catheter care, phlebotomy, and use of medical equipment like Hoyer lifts.
- Ability to work effectively with individuals with disabilities or developmental challenges across settings such as behavioral health or pediatric care.
