Ensures patient records are accurate and up to date including patient name, address, date of injury, payer information, etc. by verifying existing information or entering updated data into the computer system. 3. Collects copays and outstanding balances at the time of service. Performs daily payment reconciliation. 4. Answers incoming internal and external calls. 5. Utilizes electronic health record for messaging to/from clinical staff. 6. Enters patient information into practice management system from faxed referrals, phone or email and schedules appointments per clinical protocol. 7. Schedules outside diagnostic testing and secures authorization when required by insurance as directed by physician or advanced practice provider. 8. Performs Pre-Authorization tasks including managing ongoing authorizations for future visits when required by insurance. 9. Manages incoming faxes and other documents including but not limited to disability forms, referrals, and medical record requests. 10. Scans medical documentation and other required documents into electronic health record or other document imaging system(s) 11. The Registered Nurse provides general advisory, educational, and guidance services to our triage clients within the nurse advice line.
Essential Functions:

Performs individualized assessments and designs care plans to ensure goal achievement
</li>
<li>
Completes the patient’s medical record, including health-related aspects
</li>
<li>
Collects lab results, medication lists, and information from the primary care physician
</li>
<li>
Provides health-related education and coaching in accordance with clinical guidelines, laws, and established regulations
</li>
<li>
Promotes behavioral changes in patients through coaching, using motivational interviewing techniques
</li>
<li>
Refers patients based on specialized needs, such as mental health, nutritional assessment, medication therapy management, smoking cessation, and others
</li>
<li>
Records new patient data and interventions in the provided software
</li>
<li>
Maintains effective communication between Program team members, health plans, and professionals
</li>
<li>
Participates in educational meetings and case discussions
</li>
<li>
Ensures confidentiality and compliance with all applicable state and federal regulations related to Health Call Centers, including HIPAA and internal policies for the disposal of confidential information

Immediately reports any exposure of protected health information to the supervisor

Completes core and functional training, as well as all procedures required by regulatory agencies, contractual requirements, and applicable certification standards (including, but not limited to, URAC)

Requirements:

Associate Degree with 5 years of clinical experience and/or Bachelor's Degree in Nursing with 3 years of clinical experience.

Required Credentials: Active nursing license to practice in Puerto Rico, registration, and membership with no restrictions

Advanced knowledge of computer applications and/or Microsoft Office
</li>
<li>
Strong orientation and commitment to customer service
</li>
<li>
Excellent bilingual (Spanish and English) verbal and written communication skills – Preferred
</li>
<li>
Fast documentation and internet navigation skills
</li>
<li>
Availability for rotating shifts (including weekends/holidays), Monday through Sunday, 24/7. Shift differential pay is provided
