As a SUD Phone Counselor, you will provide support to callers across Baltimore City, Baltimore County, Howard County, and Carroll County. These services may include supportive counseling, information and referrals, crisis intervention counseling, suicide prevention and intake screening of callers to the Mobile Teams, In Home Intervention Teams, Urgent Care Center and other services. This also includes dispatching appropriate services when needed as well as providing goal directed counseling, crisis planning, and follow up for clients served. 
What You’ll Do:

<li>Triage incoming crisis calls and obtain client information.</li>
<li>Conduct assessments and dispatch appropriate resources when needed.</li>
<li>Deescalate clients in crisis over the phone.</li>

What We Require:

Job Description:
As a Pharmacist, you’ll be involved with multiple facets of the home infusion process as you interact with insurance companies, staff members, distributors, Physicians/Nurses, and patients to ensure accurate and safe quality service. Primary Duties:

<li>Coordinates and facilitates new referral/admissions through collaboration with nursing, customer service, and distribution. </li>
<li>Receives, reviews, and verifies all new patient orders and documents are the same on the order form. </li>
<li>Assists the physician and other healthcare professionals involved in planning and implementing patient care in the home to ensure safe and effective drug therapy. </li>
<li>Conducts initial and ongoing assessment of specific patient, drug, and disease information to identify real and potential drug-related problems or needs. </li>
<li>Assists the physician and nursing staff in setting therapeutic objectives for drug therapy and establishing parameters for monitoring the safety and efficacy of drug therapy. </li>
<li>Monitors the outcomes from drug therapy by collecting, evaluating, and interpreting the results of laboratory data, clinical data, and other pertinent patient information. </li>
<li>Communicates the results of therapeutic drug monitoring activities to the physician, nursing, and other appropriate persons. </li>
<li>Programs infusion control devices as indicated. </li>
<li>Provides consultation and answers inquiries from patients, families, physicians, and other healthcare professionals regarding drug and their use. </li>
<li>Will serve as designated person for all sterile compounding activities, if applicable</li>
<li>Maintains thorough and appropriate documentation in the patient/client record of all clinical activities and communication with patients, physicians, and other healthcare professionals. </li>
<li>Participates in the preparation of progress reports for the referring physician on a regular, periodic basis. </li>

Qualifications: (Education, Certification/Licensure and Experience/Skills):

<li>A Bachelor of Science in Pharmacy (RPh) or Doctor of Pharmacy (PharmD) from an accredited (Council on Pharmaceutical Education) college of Pharmacy. </li>
<li>Currently Licensed as a registered pharmacist in the state of employment. </li>
<li>Possess a minimum of two years recent experience in an acute-care setting or one-year recent experience in home infusion therapy. </li>
<li>Possess experience and strong knowledge base in intravenous admixture services and intravenous therapy. </li>
<li>Must be capable of assuming responsibility for evaluating and monitoring drug therapy relative to the total needs/problems of the home care patient in collaboration with physicians, nurses, and other healthcare professionals. </li>
<li>Can work effectively with others to provide safe and effective drug therapy. </li>
<li>Can communicate, both verbally and written, with patients, clients, management, and other employees. </li>
<li>Can problem solve and supervise technical and other support staff. </li>
<li>Must be flexible with work schedule. </li>
<li>Possess good organizational and time-management skills. </li>
<li>Participation and membership in professional societies and organizations is desirable. </li>
<li>Dependable in attendance and job performance. </li>

Essential Functions &amp; Responsibilities
