Essential Job Functions:&nbsp;

Critical Care Services ED/ICU are supported with a Charge RN 24/7. ER MD 24/7 in support with an additional ER provider mid day. All shift are 12 hour shifts every third weekend. 
JOB SUMMARY: A clinically competent, registered nurse who has demonstrated expertise in critical care. Effectively delivers critical care to patients through the nursing process of assessment, planning, intervention, implementation, and evaluation. This position is responsible to provide timely, accurate and professional nursing care to patients and families in the department.
JOB DUTIES:
1. JOB SUMMARY: This position serves the hospital patient population by observation and monitors cardiac rhythms/capnography, recognize changes in regularity, configuration and rate of rhythm and report findings to primary nurse. Appropriately documents and communicates monitored recordings. This position also includes clerical functions and specific patient care duties to facilitate efficient and comprehensive services and patient care in critical care areas. The Nurse Director/House Supervisor may re-assign to other units based on changes of volumes and acuity.
JOB DUTIES: Clerical: 1. Accurately identifies and enters the patient information: initials, telemetry box number into the monitor system and telemetry/capnography log. 2. Assembles/disassembles patient shadow charts in a timely fashion to facilitate patient flow. 3. Promptly and accurately responds to delegated tasks within scope of practice. 4. During EHR downtime, accurately and promptly completes necessary paperwork which assists to ensure tests, consults and treatments are initiated/completed. 5. Reports appropriate overhead pages via the paging system to the Admitting staff in accordance with established organizational procedures. 6. Completes after visit summary (AVS) and any additional discharge or transfer process. 7. Maintains a clean and organized workstation. 8. Identifies all patient information using HIPAA standards. Patient Care Duties: 1. Assists in the delivery of patient care as directed to include: transfers, ambulation, feeding, dressing, bathing, weighing, and toileting. 2. Promptly delivers specimens and supplies within the hospital. 3. Responds promptly to all patient safety alarms, i.e. call lights, bed and chair alarms. 4. Notifies primary nurse immediately of significant changes in monitor patterns and follows the chain of command if no response or inappropriate response occurs. 8. Accurately recognizes and reports life threatening arrhythmias (Ventricular Tachycardia, Ventricular Fibrillation, Asystole). 9. Accurately interprets and measures intervals of the cardiac rhythm and other parameters upon admission, at the beginning of each shift, and with significant changes. 10. Advocates for the patient by ensuring appropriate alarms are on and limits are set - individualized for the patient in collaboration with the RN or physician or defers to default settings when appropriate. 11. Alert to alarms when not directly in eye sight of monitor while initiation NA duties. 12. Advocates for patient safety by troubleshooting problems at the bedside; performing battery replacement, ensuring the replacement of electrodes to achieve the best possible waveform in a timely manner. 13. Demonstrates accurate interpretation of monitors including appropriate documentation and communication of monitored recordings to the primary nurse. 14. Provides the primary nurse with a printout of a rhythm strip (with a clear baseline) on admission, at the beginning of each shift, and with significant change in rhythm for documentation in the patient's medical record. General: 1. Demonstrates knowledge of and compliance with hospital safety, disaster and emergency procedures. 2. Cleans and disinfects equipment as needed keeping unit clean and organized. 3. Attends required in-services and a minimum of 50% yearly attendance at unit staff meetings. 4. Understands and adheres to departmental policies and procedures. 5. Care is delivered with adherence to the policies and procedures of Grand Itasca Clinic and Hospital. 6. Performs and demonstrates competency of CNA responsibilities for patient care. 7. QUALIFICATIONS:

Organizational Responsibilities:

Individual Receiving Services Care Responsibilities:
• Does not disclose individual receiving services’ protected health information and promptly report suspected or known violations of such disclosure to the Administrator. • Ensure that all care is provided in privacy, as appropriate, and to knock after entering an individual receiving services’ room or individual receiving services care area such as tub rooms. • Report all grievances and complaints made by the individual receiving services, family, or visitors to the appropriate supervisor following the care center’s established procedures. • Report to licensed staff any necessary observations made regarding potential changes in individual receiving services status, utilizing the STOP AND WATCH tool. • Participate in and receive the nursing report upon reporting to duty. • Follow written NAR procedure for designated shift. • Attend to daily care needs per each individual receiving services’ plan of care (e.g. bathing, oral care, hair care, nail care, transfers, feeding, ambulation, R.O.M., elimination needs, intake, output, catheter care, dressing, turning and repositioning, collection of specimens, weights and taking vital signs) and document the care each time it is provided. • Follow established policies/procedures for performing all cares. • Provide considerate and respectful care, preserving the personal dignity, modesty and privacy of the individual receiving services and family. • Turn all medications found in the individual receiving services’ room/possession (that are not care planned to be there) over to the Director of Nursing Services or Nurse Manager. • Observe individual receiving services regularly and respond promptly to call lights. • Report changes in individual receiving services’ condition and/or normal routine including nutrition, elimination, ambulation and skin integrity. • Communicate (intra shift) with co-workers, supervisor, and DON as appropriate, regarding individual receiving services’ progress and your progress with daily individual receiving services’ care.

• Keep the call light system within easy reach of the individual receiving services. • Demonstrate and practice correct sanitation techniques, such as hand washing, proper handling of supplies, cleansing and sanitizing equipment, per policy procedure. • Participate in appropriate in-service training programs prior to performing tasks that involve potential exposure to blood/body fluids or hazardous chemicals. • Use personal protective clothing/devices/equipment when handling infectious waste and/or blood/body fluids. • Follow established policies and procedures in performance of duties such as safety, infection control and exposure to blood/body fluids. • Adhere to basic infection control practices, and use standard precautions. • Follow care center’s established policies governing safety data sheets (SDS) and the handling of hazardous chemicals.
