Patient Care Technician (PCT) 
Patient Care Technicians (PCTs) at Health Systems Management support and work under the guidance of RNs to provide dialysis treatment to multiple patients at a time. PCTs have a combination of technical skills and compassion needed to improve the quality of life for our patients while promoting positive patient outcomes. Successful candidates will have the opportunity to learn the discipline of dialysis. To be considered for this position qualified candidates are required to have a minimum of 6 months of healthcare and/or direct patient care experience. Good fit for those with - CHT, CCHT, CNA, CMA, PCA, CPT, CMA, EMT, CST, CPhT, etc.

PCT Responsibilities and Physical Demands:
The physical demands and work environment characteristics described here are representative of those an employee encounters while performing the essential functions of the job.

<li>Set up of the hemodialysis machine. </li>
<li>Greet patients and collect details regarding their previous treatment. Report any concerns or observations to the lead nurse. </li>
<li>Assist with and prepare the patient's vascular access for treatment. </li>
<li>Initiate, monitor, and discontinue dialysis treatment per written policies, procedures, and standards of care. </li>
<li>Obtain patient's vital signs before, during, and after dialysis treatment and appropriately document in patient medical record. </li>
<li>Responsible for appropriately calculating and documenting patient's fluid removal. </li>
<li>PCT performs appropriate access cannulation and reports any difficulties or unusual findings to the lead nurse. </li>
<li>Monitor the patient for any adverse reaction(s) and conduct appropriate intervention.</li>
<li>Monitor the dialysis machine for any malfunction. Report any malfunctions to the lead nurse. </li>
<li>Demonstrates knowledge of infection control as related to hand washing, personal protective equipment (PPE), and universal precautions. </li>
<li>Maintain patient safety and comfort, which may include assisting with personal care needs and activities of daily living. </li>
<li>Responds appropriately to emergency situations. </li>
<li>Note you will have exposure to blood, needles, and various other bodily fluids. </li>
<li>This position provides direct patient care that regularly involves heavy lifting, moving of patients, and assisting with ambulation. </li>
<li>PCT will spend considerable time walking, bending, stretching, and standing. </li>
<li>The employee may occasionally be required to move machines, equipment, and may lift chemical and water solutions of up to 30 pounds and high as five feet. </li>
<li>You are required to acknowledge the inter-facility coverage policy that supports floating to various clinics during and after your training, if needed. </li>

PCT What You Can Expect: 
<ul>
<li>Improvement: HSM consistently strives to improve processes for our patients and teammates.</li>
<li>Training: May take plate in a facility or a different clinic other than your assigned home clinic. </li>
<li>Hours: Work hours may vary during the training period. However, you will typically work between 36-40 hours per week during training. </li>
</ul>
Education Requirements and Position Qualifications: 
<ul>
<li>High School Diploma required.</li>
<li>Basic computer knowledge required.</li>
<li>6 Months of healthcare and/or direct patient care experience is required.</li>
<li>Continued employment is dependent on successful completion of the Health Systems Management training program in its entirety. </li>
<li>The ability to work long hours is required. You could be on your feet for extended periods of time. Our day ends when patients’ care is completed.</li>
<li>You must have a flexible schedule and be able to work mornings, evenings, weekends, and holidays. </li>
<li>Must pass the Certified Clinical Hemodialysis Technician (CCHT) exam within 18 months following the first day of employment.</li>
<li>Must have or obtain Basic Life Support certification approved by the American Heart Association</li>
<li>Must be comfortable working around blood and needles, including cannulation, and obtaining labs.</li>
<li>Excellent bedside manner and communication skills. </li>
<li>Employee must successfully pass the Ishihara’s Color Blind Test.</li>

The Sleep Technologist performs various diagnostic and treatment polysomnograms on adolescents and adult patient populations in addition to other sleep disorder tests in accordance with policies and procedures. Monitors the patient throughout the sleep study and documents observations, collects data from various monitors and reports this information to the interpreting physician.

Responsibilities
As a Home Health Clinical Manager, You will:     
<ul>
<li>Conducts/participates in quarterly quality improvement meetings, reviews pertinent available data, identifies trends, and oversees the implementation of improvement plans </li>
<li>Collaborates with the Branch Director in the implementation of quality improvement activities and corrective action plans </li>
<li>Responsible for the oversight of and response to external and internal surveys and audits within the assigned area relating to the quality of care and clinically related KPIs in collaboration with operations leadership </li>
<li>Completes documentation and chart reviews to identify variances in standardized care and care processes and to identify areas for improved performance related to patient care and the organization </li>
<li>Holds case conferences to ensure oversight of care, coordination of services and that standards are met </li>
<li>Facilitates the development of performance improvement action plans, based on available data (e.g., survey results, QAPI and clinical record reviews). Accomplishes staff results by communicating job expectations; planning, monitoring, and appraising job results; coaching, counseling, and disciplining employees; developing, coordinating, and enforcing systems, policies, procedures, and productivity standards </li>
<li>Ensures assigned staff are evaluated fairly and timely in accordance with company performance evaluation procedures </li>

Qualifications
<ul>
<li>Bachelor’s Degree preferred in Nursing from an accredited school of nursing</li>
<li>Licensed Registered Nurse (RN) in good standing and currently licensed by the State </li>
<li>Less than two years of clinical experience, and at least one year in a related management/supervisory role in home health care </li>
<li>Current CPR certification </li>
<li>Must meet all agency requirements for pre-employment as required by the company and/or State regulations </li>
<li>Ability to use company documentation systems </li>
<li>Ability to communicate (verbally and written) with all levels of personnel, internal and external to the company </li>
<li>Ability to work independently as well as part of a team </li>
<li>Capable of working responsibly with highly confidential information </li>
