Pathways to Housing DC (Pathways DC) is an innovative agency based in the heart of Washington, D.C. We believe that housing is a basic human right, and that without housing people are unable to address their mental health disorders, addictions, and other life stressors. We provide each person entering our program their own apartment, without requiring treatment compliance or sobriety as a prerequisite. We then provide each person with intensive support services to help them stay housed and begin their journey to recovery.
Under the direction of the Associate Director of Homeless Operations, the DDSC Lead Case Manager is responsible for the daily operation and supervision of DDSC housing services. The Lead Case Manager will help coordinate a team of case managers, ensuring effective service delivery. They will be responsible for assuring that the agency values and philosophy of consumer choice and respectful concern are expressed in all aspects of team practice. They will serve as the liaison for case coordination with Pathways and other providers to ensure timely and accurate sharing of information. This role requires strong collaboration and leadership skills, as well as a deep understanding of homelessness issues and the ability to engage with vulnerable populations.

Responsibilities:
Co-Lead Coordination

<li>Collaborate with Associate Director of Homeless Operations to ensure coordination and operation of case management team</li>
<li>Share responsibilities for team management, training, task assignments, and performance evaluation.</li>

Qualifications

The NA provides patient-centered nursing care and daily living assistance to assigned patient under the supervision of a Registered Nurse (RN) or Licensed Practical Nurse (LPN). Provide assistance with basic health care needs including daily living activities; bathing, grooming, toileting, eating, meal preparation and dining needs, emotional and spiritual needs. Administer delegated tasks and treatments. Record vital statistics and other basic health information. Will collect non-invasive biological specimens from patients as needed. Perform clerical work related to scheduling and documenting patient healthcare. Prepare patient treatment areas for use; prepare and clean medical equipment; dispose of biomedical waste in accordance with standards and policy. Clean patient living areas or treatment rooms. Qualifications Must be able to acquire a basic knowledge of patient care principles, medical terminology and equipment within the assigned service area. Certification and registration with the Minnesota Board of Nursing Assistant Registry is required. Certification is required if designated by law (i.e. Home Care).
