<li>Coordinate patient scheduling process to maximize doctor and hygiene production, coordinate incoming patient flow with clinical staff.</li>

<li>Conduct new patient exam, take initial photographs and X-ray. Communicate new patient concerns to doctor during initial consult prior to doctor meeting patient.</li>
<li>Make follow up telephone calls to prospective patients following initial meeting to those that have not decided.</li>
<li>Follow up with referring dentist to answer questions related to X-rays, proposed treatment plan, etc.</li>
<li>Prepare referral letters on behalf of the Doctors for referring doctors, parents and/or patients.</li>
<li>Compile monthly new patient report including new patients, starting records and starts.</li>
<li>Exercise good teamwork and partner with team to create good patient experience.</li>
<li>Routinely assist with administrative duties.</li>
<li>Participates in the medical office emergency routine when required. Summons ambulance or EMS or assists other staff members as needed.</li>
<li>Navigate software without assistance.</li>
<li>Must always represent the practice in a professional, pleasant, and cooperative manner.</li>
<li>Maintain regular attendance and adhere to assigned work schedules and office policies.</li>

The PT communicates and collaborates with families, physicians and other health team members and maintains impeccable, professional documentation of services in the medical record. The PT also conducts engaging in-services and training for facility staff on an ongoing basis in restorative and functional maintenance programs. The PT reports to the Therapy Program Manager and ultimately to the Director of Operations. CRI’s PTs model and promote competent, courteous and professional services to internal and external customers to develop and maintain a competitive advantage. CRI PTs maintain career development through continuing education and training opportunities and networking. Requirements:
ESSENTIAL FUNCTIONS OF THE JOB
<ul>
<li>Evaluate Patients for rehabilitation services within 24 to 48 hours of physician’s orders.</li>
<li>Develop effective treatment plans.</li>
<li>Treat patients according to the treatment plan of care.</li>
<li>Supervise the Licensed Physical Therapy Assistant in the delivery of clinical services and interactions with other staff, patients and family members.</li>
<li>Communicate with physicians, clinical managers, and other health team members, patients, family and facility staff regarding patient progress, problems and plans.</li>
<li>Participate in Plan of Care Meetings upon request and Interdisciplinary Team Meetings.</li>
<li>Achieve or exceed the minimum productivity standard established by CRI.</li>
<li>Participate in in-service training programs for other staff in the facility.</li>
<li>Record treatment notes, progress notes, provide objective input for discharge summaries, and complete daily billing of services per policy and procedure.</li>
<li>Instruct patient’s families and/or facility staff in maintenance programs.</li>
<li>Participate in discharge planning.</li>
<li>Recommend any needed equipment to the appropriate personnel which would serve to facilitate improved delivery of clinical service.</li>
<li>Report any problems with department equipment to the facility personnel.</li>
<li>Act as a clinical supervisor for affiliating Physical Therapy and Physical Therapy Assistant programs for area universities and colleges.</li>
<li>Periodically work beyond usual working hours as needed for facility emergencies, weekends and holidays.</li>
<li>Consistently portray the mission, vision, core values, cornerstones and professional image of Concept Rehab, exercise good judgment in the performance of the job.</li>
<li>Special projects and other duties as assigned.</li>

REQUIRED QUALIFICATIONS

<li>B.S. Masters or DPT in Physical Therapy.</li>
<li>Licensed in the state(s) where services are being provided as a Physical Therapist.</li>
