<li>Updates Electronic Infection Log in eMar as needed with all pertinent information. Notify Infection Control Preventionist of any new infections.</li>

Our teams work to address each individual on a holistic level so they feel heard, respected, and safe in our care. We offer hands-on help to address chronic homelessness, mental health, substance abuse, disability, and systematic poverty&nbsp;by offering innovative solutions and proactive services to the most vulnerable in Schenectady County. Our Case Managers facilitate services across all spectrums of needs:

<li>Financial Planning and Budgeting</li>

</ul>
Individual case management responsibilities vary across the team based on the assigned area of focus. Our Case Management Services generally include:
<ul>
<li>Emergency/Crisis Assistance – primarily assists homeless adult individuals and occasionally, families navigate the complex system of care to receive emergency shelter placement and other basic needs as quickly as possible.</li>
<li>Housing/Outreach Case Management – assists guest to stabilize income and find the most suitable housing that is both safe and affordable. Includes renting from reputable landlords, applying for Section 8 Housing, Municipal Housing or other housing programs.</li>
<li>Representative Payee Financial Management Program – manages SSI and SSD benefits for disabled guests particularly those with a history of chronic homelessness in spite of their steady monthly income. These individuals are often easily victimized and lack the ability to make sound financial decisions. Bethesda House Case Managers work with each guest to establish a budget to ensure that rent, food, medical care and other basic needs are met prior to the guest being given a personal spending allowance. This program has ended the cycle of chronic homelessness for every participant who was homeless at the time of entry.</li>

<li>3-5 years of professional, case management experience providing services to individuals with chronic medical conditions and behavioral health issues or equivalent background.</li>

DUTIES
<ul>
<li>Conduct home visits and phone contacts with the dual purpose of assessing the safety and appropriateness of the child's placement with their caregiver (sponsor) and connecting the child to services in the community to address risk factors.</li>
<li>Ongoing case management with a focus on screening and making appropriate referrals to address child's needs in the following domains: Placement issues, safety issues, education, health, mental health, legal/immigration proceedings, substance abuse, independent living, criminal/juvenile justice, and child enrichment activities.</li>
<li>Provide crisis intervention services directly, as needed, or connect the family to services to ensure the minor's safety and well-being in a crisis situation.</li>
<li>Submit detailed and thorough documentation in compliance with Global Refuge policies and procedures.</li>
<li>Utilize social work best practices for thorough assessments of the child's progress and services provided.</li>
<li>Maintain regular and timely communication with the supervisor around any issues concerning the child's safety, stability, or well-being in the placement.</li>

QUALIFICATIONS
