We are presently seeking a Groundskeeper for our Windsor Estates community located in West Valley City, UT, who will perform general grounds and lawn maintenance.
Responsibilities will include mowing lawns, weeding flowerbeds, cutting weeds, mulching, and other applicable landscaping tasks.&nbsp;Seasonal maintenance tasks may also required; such as salting walkways, as well as snow and ice removal.

Job Summary
Collaborates with the physician and healthcare team in the delivery of patient care, utilizing the nursing process. Provides professional nursing care to patients, including conducting nursing assessments, assisting with exams and treatment, patient education, and maintenance of medical records. MHHS hires RNs based on years of experience groupings (i.e. newly licensed RN [working under the supervision of RN clinical preceptor], less than 1 year, 1-3 years, 3 to 10 and 10+).Job Description
Minimum Qualifications
Education: Graduate of an accredited School of Professional Nursing; Bachelors of Nursing preferred
Licenses/Certifications:

Summary/Objective:
Primary job responsibilities involve service linkage and care coordination to assist consumers in addressing their behavioral health needs and tracking relevant health outcomes. Case Managers will provide community support, case management and service linkage both within Boundless and with outside providers that identify and address behavioral health needs, developmental issues and barriers to care. The Care Coordinator will work with consumers, parents/guardians, Boundless staff and outside providers that support treatment and contribute meaningfully to whole health outcome of the individual served. 
Essential Functions:

<li>Provides case management support to consumers engaged in behavioral health services including community psychiatric support and therapeutic support to promote the coordination and implementation of treatment goals   </li>

<li>Serve as a liaison between families and agency to positively promote collaborative processes in the treatment planning for clients by encouraging parent/guardian participation in treatment. </li>
<li>Identify and assess psychosocial and life skills needs through analysis of factors impinging on client success including factors in the home, school and community. </li>
<li>Provide parent/guardian educational workshops on identified issues related to child development, stress reduction, discipline and safety, and teacher/parent/client communication. </li>
<li>Keep caseload spreadsheet (information management) updated regularly.  </li>
<li>Manage phone calls from families, guardians, agencies, etc. Set up meetings; obtain information, etc.  </li>

Education/Experience:
