ASHP-accredited post-graduate year one residency training program (PGY1) with Board of Pharmacy Specialties (BPS) board certification.OR ASHP – accredited post -graduate year two (PGY2)OR ASHP – accredited PGY-1 with three (3) years of clinical practice experience in with Board of Pharmacy Specialties (BPS) board certification.Equivalency Determination Request: The Contractor shall not request an equivalency determination as a substitute for education requirements on this labor category.Certifications (in addition to Basic Life Support identified in paragraph 1.5.45.6.): American Heart Association Advanced Cardiovascular Life Support (ACLS)certification for all acute care pharmacists (i.e. critical care, cardiology, emergency medicine, internal medicine)Board Certification: of Pharmacy Specialties (BPS) Board CertificationLicensure : Current, full, active, and unrestricted license to practice as a clinical pharmacist.Contract Worker Standard Qualifications : (see base contract PWS 1.5.45.1.). The standard qualifications (duties) or the labor category are as follows:Experience: All hospital clinical experience must be recent within the past 2 years post-graduation/training. Candidate must meet standards of an ASHP-Qualified Residency Preceptor within one year of employment.Clinical Pharmacist - CardiologyProvides direct patient care, including treatment assessment and medication management activities.Works collaboratively with multidisciplinary teams to optimize drug therapy.Clinically evaluates all patients in specified area daily with relevant documentation placed in the medical record regarding pharmacy recommendations, plans, interventions, and medication reconciliation.Reviews the patient's drug regimen for appropriate indication, dosing allergy verification, interactions potential, lab monitoring, duplication of therapy, drug interactions and medication documentation.Provides anticoagulation management services, including initiation, monitoring, and adjustment of warfarin, direct oral anticoagulants, and heparin, based on established protocols and guidelines. Educates patients on anticoagulation therapy and bleeding risks.Recommends and monitors optimal dosing strategies for cardiovascular medications.Provides comprehensive Medication Therapy Management services to cardiology patients, including medication review, adherence counseling, and disease state management.Provides cost-effective management of drug utilization within the clinic, monitors process and outcome of patient care relating to drug therapy, monitors polypharmacy and high dose pharmaceutical dosing to prevent adverse effectsClinical Pharmacists provide education on pharmaceuticals to BAMC staff and beneficiaries as requested and in support of residency requirements for rotation opportunities.Work Schedule:The Contractor shall be available to provide support (Monday through Friday) from (0800-1700) daily. Contract Personnel shall be available to provide support (Sunday through Saturday) from (0001-2400) daily.The Department shall establish a schedule for contract personnel to perform services on various shifts, which range from (8-12) hours. Each shift shall include (30 or 60) minutes for unpaid lunch, which may vary depending on the section and/or specific shift assigned.Regular duty hours are expected to average (40) hours per week. Unplanned or unannounced schedule changes may occur, based on necessity, and in concurrence of the Contractor.Clinical pharmacists may rotate to clinical positions provided the clinical pharmacist is trained and competent within the disciplineClinical Pharmacists: according to clinic schedule between the hours of 0530-2100.When directed by the Government, contract personnel shall remain on-duty to complete patient treatment, where lack of continuity of care would otherwise jeopardize patient health. This provision is not intended to apply to the time required to complete routine tasks (e.g., completion of paperwork or routine administrative tasks at the end of a shift) which are to be completed as part of the normal work schedule.
