Job DescriptionPhysical Therapist (PT)Role Summary

Are you an inspirational, community-minded clinical leader who is passionate about helping vulnerable seniors regain their safety, mobility, and independence? We are seeking an adaptable and highly collaborative Physical Therapist (PT) for a full-time contract position in San Jose.

In this dual clinical and management role, you will coordinate and deliver vital restorative and rehabilitative physical therapy services to seniors both at our day health center and in their homes or residential care facilities. You will partner closely with staff Physicians, Occupational Therapists, and an interdisciplinary team (IDT) to optimize participant safety, balance, and performance of Activities of Daily Living (ADLs). Additionally, you will serve as a key member of our management team, directly supervising and mentoring Therapy Assistants and guiding them in executing Functional Maintenance Programs (FMPs). Duties & ResponsibilitiesClinical Assessment & Therapy Delivery

  • Comprehensive Assessments: Conduct physical therapy evaluations to identify participants' therapeutic needs, preferences, and goals, actively contributing to IDT meetings to formulate individualized care plans.

  • Therapeutic Interventions: Deliver and document physician-ordered physical therapy interventions. These include activities designed to improve active/passive exercise, muscle re-education, gait and functional ADL training, and specialized treatments (such as massage, heat, sound, water, light, or electricity).

  • Equipment & Transition Management: Order, track, and manage routine maintenance for therapeutic equipment. Support smooth care transitions between hospitals, Skilled Nursing Facilities (SNFs), and homes, and assist with end-of-life care coordination.

  • Enrollment Support: Coordinate with marketing teams to support the enrollment of prospective participants into the program.

  • Clinical Supervision: Direct, train, and instruct Therapy Assistants in executing Functional Maintenance Programs (FMPs).

  • Mentorship & Team Leadership: Manage, supervise, and mentor direct reports, utilizing operational reports and data to prioritize workloads and remove team barriers.

  • Caregiver Education: Provide hands-on training and instruction to staff, participants, and family caregivers in the use and care of therapeutic equipment.

  • Other duties as assigned.

Required QualificationsEducation, Licensure & Credentials

  • Education: Bachelor’s Degree in Physical Therapy from an accredited program (qualifying physical therapy degrees earned outside of the US must be evaluated to the US equivalent).

  • Licensure: Valid California Physical Therapy License.

  • Life Support: Active Basic Life Support (BLS) certification.

Experience & Skills

  • Progressive Experience: Minimum of five (5) years of progressively responsible physical therapy experience.

  • Clinical Specialty: Minimum of two (2) years of clinical Physical Therapy experience working directly with a frail or elderly population.

  • Supervisory Experience: Minimum of two (2) years of direct supervisory or management experience, with a proven ability to mentor, train, and develop clinical team members.

  • Mobility: Must possess reliable transportation to complete required clinic-facing and field-based mobile therapy visits.

  • Work Authorization: Must be legally authorized to practice and work in the State of California.

Desired Qualifications

  • Advanced Degree: Master’s or Doctorate Degree in Physical Therapy (DPT) is preferred.

  • Data-Driven Management: Prior experience working in a data-driven healthcare environment, leveraging reporting metrics to manage teams and prioritize assignments.

  • Adaptability: Exceptional communication and organizational skills, with a proven ability to work independently, manage shifting priorities, and collaborate with diverse cultural groups.

Location and Work Type

  • Work Type: Full-Time Contract Position (40 hours per week).

  • Work Setting: Hybrid clinic-facing and field-based setting. Based out of the center in San Jose, CA with required local mobile travel to patient residences, SNFs, or ALFs.

  • Schedule: Monday through Friday, 8:00 AM – 4:30 PM.
