Minimum Qualifications&nbsp;

Job Description *This is a pool role Essential Job Duties:

<li>Conducts initial and ongoing assessment of specific patient, drug and disease information to identify real and potential drug-related problems or needs. Completes patient compounding record, labels final product. </li>
<li>Monitors the outcomes from drug therapy by collecting, evaluating and interpreting the results of laboratory data, clinical data other pertinent patient information.</li>
<li>Maintains thorough documentation in the patient/client record with patients, physicians and other healthcare professionals. Clinical skills and knowledge of pharmacotherapy by attending in-services and presenting journal clubs to staff. Performs data entry to refill prescriptions by printing labels, compounding records, delivery tickets and related scheduling documentation.</li>
<li>Communicates with other departments as required for notification of patient status changes. Documents communications and related changes as necessary in pharmacy computer system.</li>
<li>Compounds sterile preparations in accordance with all USP 797 and Pentec Health policies and procedures.</li>
<li>Work a minimum of every other Sunday for 8 hours throughout the entire year.</li>
