Requirements

<li>Possession of current license to practice physical therapy in the state of Connecticut and/or Florida</li>

<li>Active membership in the APTA(national, state, and orthopedic section)</li>

Job Responsibilities for a Licensed Clinical Social Worker (LCSW):
<ul>
<li>Administer Psychological Evaluations in home and/or school based settings</li>
<li>Gather necessary information to conduct social histories.</li>
<li>Write comprehensive evaluation reports and social histories and submit them in a timely manner</li>
<li>Write Individualized Educational Plan (IEP) and/or Individualized Family Service Plan (IFSP) goal</li>
<li>Determine need for additional assessment per observation and child need</li>
<li>Conduct evaluations in the primary language of the child</li>
<li>Maintain child records and confidentiality of information regarding children and families (in accordance with HIPAA, state and federal regulations). Required records must be submitted within the established timelines.</li>

Requirements for a Licensed Clinical Social Worker (LCSW):
<ul>
<li>Must have a passion for working with children and families!</li>
<li>NYS Licensed Clinical Social Worker (LCSW).</li>
<li>Experience evaluating children birth - 5preferred but not required.</li>
<li>Familiar with using evaluation tools such as ADOS 2 or CARS 2.</li>
<li>EI Evaluations program requires completion of the EILN Intro and Advanced Assessment trainings.</li>
<li>Must be self-motivated and a team player who exercises patience and professionalism.</li>
<li>Fluency in a language other than English is a plus!</li>
</ul>
Physical Requirements:
<ul>
<li>Must be able to travel to and from assigned cases, including but not limited to:</li>
<li>Driving a personal vehicle, walking and/or the use of public transportation such as a bus or train.</li>
<li>The ability to ascend and descend staircases.</li>
<li>The ability to lift 10 pounds regularly.</li>
<li>The ability to sit on the floor, kneel and/or crawl for extended periods of time.</li>
