Responsibilities
What to Expect:
<ul>
<li>Conduct Adult/Geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols</li>
<li>Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate</li>
<li>Collaborate with Primary Care Physician (PCP) on patient education, provide follow-up</li>
<li>Provide services in a variety of venues to include: Home Visits, Skilled Facility Visits</li>

Nurse Practitioner or Physician Assistant - PRN - Home Risk Assessments
Type: PRN - Flexible
PAY: Earn from $2,300 up to $11,500 per month (Based on an average of 20-100 completed visits per month, and pay of $115.00 per completed assessment). Compensation range is based on availability and assessment completion.
Location: Home, SNF and other community environments in Rego Park, New York
Hours: Days, Evening, Nights, weekend, Flexible Scheduling
Benefits: PRN providers are non-benefited except for 401(k) and limited Voluntary Benefits

Qualifications
Must Haves:
<ul>
<li>Master’s Degree required OR commensurate experience and satisfactory completion of NP licensure</li>
<li>Master’s Degree required OR commensurate experience and satisfactory completion of PA licensure</li>
<li>Current RN and NP or PA licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority</li>
<li>Board certified by the NCCPA, AANP, ANCC or the AACCN in a Matrix approved specialty</li>
<li>Have a current BLS, ACLS or CPR certification</li>
<li>1 year experience as a Nurse Practitioner/Physician Assistant preferred, new grads encouraged to apply. Have the ability to travel either in state or out of state</li>
<li>Have strong computer skills and familiarity with EMR software</li>
<li>Possess excellent verbal and written communication skills with patient, clients and colleagues</li>
<li>Have a positive and outgoing personality, and be a team player. Be comfortable and flexible with frequent change</li>

<li>Ability to travel - Valid state driver license, able to drive a car, proof of adequate automobile insurance coverage for the state of residence</li>

Key Responsibilities:

<li>Perform routine dental cleanings, including scaling, root planing, and polishing teeth to remove plaque and tartar buildup.</li>
<li>Assist with patient education on proper oral hygiene techniques, diet, and preventive care to maintain healthy teeth and gums.</li>
<li>Administer local anesthesia and perform sealant application for patients as required.</li>
<li>Take and develop dental radiographs (X-rays), ensuring proper film processing and adhering to safety protocols.</li>
<li>Collaborate with dentists to evaluate patients' oral health, identifying signs of potential concerns and assisting in treatment planning.</li>

<li>Maintain accurate patient records, documenting hygiene procedures, treatment recommendations, and patient progress.</li>

Qualifications
<ul>
<li>Associate’s degree in Dental Hygiene from an accredited dental hygiene program.</li>
<li>Licensed to practice dental hygiene in the state (e.g., RDH license).</li>
<li>Experience in providing comprehensive dental cleanings and preventive care.</li>
<li>Proficiency in performing routine diagnostic and preventive procedures, including X-rays, fluoride treatments, and oral cancer screenings.</li>
<li>Strong communication skills for educating patients and collaborating with the dental team.</li>
<li>Commitment to patient care, with a focus on compassion and creating a comfortable environment for patients.</li>
<li>Willingness to stay updated on the latest dental hygiene techniques, advancements, and technologies through continuing education and professional development.</li>
