Position Summary: As a Home Manager at Sunnyhill, you’ll have the incredible opportunity to lead a team and create a home where residents feel safe, supported, and truly happy. Essential Duties & Responsibilities:
- Ensure smooth daily operations of the home, maintaining a safe and clean environment, ensuring it meets the health and safety standards.
- Advocates for the people we support in all aspects of their lives.
- Assists in planning, development, and implementation of Individual Support Plans (ISPs)and Behavior Support Plans (BSPs).
- Oversee the delivery of person-centered services that meet the needs and preference of each person we support.
- Work 40 hours direct care per week in the home. Provide 24-hour on-call support, in rotation with the Lead Staff, to assigned work site and respond within one (1) hour to all phone calls.
- Develop and implement schedules for staff and the people we support to ensure adequate coverage and support to meet the individual needs of each person we support. Fill or cover direct care shifts as needed and communicate any staffing needs to management.
- Coordinate with healthcare providers, partnering agencies, and other stakeholders to address the needs of the people we support.
- Communicate positively and effectively with all people we support, employees, and stakeholders
- Respond in a timely manner to all communication, including Sunnyhill emails, at minimum daily.
- Ensure employees are adequately trained at the site before they work a shift
- Provide feedback to employees through 90-day evaluations, annual appraisals, disciplines, and ensure that all assigned staff maintain current certifications as required by agency policy.
- Accompany the people we support to their medical appointments, both scheduled and unscheduled.
- Complete and document monthly adaptive equipment checks to ensure they are in working order and immediately report any issues.
- Participate in monthly nursing and service coordinator visits a ensure the health and wellbeing of the people we support.
- Collaborate with nursing to ensure all prescribed medications and treatments are available and taken as directed by maintaining current orders and supplies. Review Medication Administration Records (MARS) twice weekly.
- Maintain accurate records and documentation for each person we support including complete and review daily notes, monitor goals and outcomes, review and submit incident reports.
- Manage and monitor expenses and ensure financial transactions are documented and reported accurately.
- Complete necessary paperwork to ensure that all people we support maintain eligibility for all applicable benefits, including food stamps, Medicaid, Medicare, Medicare prescription coverage, etc.
- Collaborate with case management entities to ensure compliance.
- Conduct monthly team meetings with all staff at assigned site.
- Attend monthly department meetings and participate in professional development training assigned.
Requirements:
Education & Experience
- High School Diploma or GED
- A direct knowledge of working in the Human Services field, with the developmentally disabled required.
- Two (2) years of related experience, preferred.
- Supervisory experience, preferred.
Required Skills/Abilities & Qualifications
- Excellent verbal and written communication.
- Ability to prioritize tasks and function well in a high-paced (at times stressful) environment.
- Valid driver’s license and auto insurance (use of personal vehicle required at times)
- Acceptable background checks
Physical Requirements
- Able to stand, walk, and sit four (4) hours at a time.
- Repetitive bending and reaching up to two (2) hours a day.
- Able to lift 25 pounds from floor to waist.
