At Mountain Valley Hospice, admissions are more than paperwork — they are the first step in building trust with patients and families during one of the most important transitions of care. We are seeking a compassionate, organized, and clinically confident Registered Nurse to serve as an Admissions RN for skilled nursing facilities (SNFs) and assisted living facilities (ALFs) in the Forsyth County area.

What You’ll Do:

As an Admissions RN, you will be responsible for coordinating and completing hospice admissions in facility settings, ensuring a smooth, timely, and supportive transition for patients, families, and facility partners.

Key responsibilities include:

  • Completing hospice admissions in SNF and ALF settings
  • Conducting thorough clinical assessments and eligibility evaluations
  • Educating patients, families, and facility staff on hospice services and expectations
  • Collaborating with referral sources, physicians, and the interdisciplinary team
  • Ensuring accurate, timely documentation to support compliance and quality care
  • Serving as a professional and compassionate representative of Mountain Valley Hospice in the community

What You’ll Bring

  • Current RN license in North Carolina (or compact state)
  • Strong clinical assessment skills and attention to detail
  • Excellent communication skills and comfort working with patients, families, and facility staff
  • Ability to work independently while collaborating closely with a team
  • Hospice or admissions experience preferred (but not required)
