Home Health RN | Details and Compensation:
- Location: Manchester, TN 37355 – 100% on-site
- Travel required between patient locations (4–6 visits/day)
- Payrate: $48.69/hr
- Required Availability: Monday – Friday, 8:00 AM – 4:30 PM + on-call 1 weekend per month
Home Health RN | Summary and Highlights:
The Home Health Registered Nurse (RN) will provide high-quality care to individuals in their homes. The ideal candidate is highly organized, technologically proficient, and capable of working independently. This is a high-skill role that includes direct patient care, care plan management, and coordination with other healthcare providers.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
Home Health RN | Responsibilities:
- Between patient locations (4–6 visits/day)
- Conduct health assessments and implement care plans
- Perform OASIS assessments
- Monitor and manage chronic or acute conditions
- Ensure health maintenance, disease prevention, and case management
- Supervise care plans and possibly staff as needed
- Coordinate multidisciplinary care and communication
- Handle medication ordering, monitoring, and administration policies
- Document daily, monthly, quarterly, and annual nursing reports
- Schedule and follow up on medical appointments
- Provide training on health and safety for new and current employees
- Communicate with hospitals and discharge planners when needed
Home Health RN | Requirements:
- Education: High School Diploma or GED (Verifiable).
- Active Tennessee RN license or Compact State RN license
- Home Health experience is required
- Proficiency with OASIS documentation
- Experience with HCHB (Homecare Homebase) HIGHLY preferred
- Strong patient relations and communication skills
- Tech-savvy and comfortable with electronic health systems
- Ability to travel between patient locations (mileage reimbursed)
- Attendance: Commitment to mandatory attendance during the first 90 days.
