Community Resource Coordinator (#169312)
Pay: $22.00 - $26.00 per hour
Schedule: 1st Shift – 8:00 AM–4:00 PM
- Hybrid + regular regional travel for meetings
Location: Norristown, PA 19403
Community Resource Coordinator Summary: The Community Resource Coordinator (CRC) supports licensed personal care homes serving low-income residents by building relationships with community organizations and connecting facilities/residents to available resources. You’ll conduct on-site needs assessments, provide technical assistance, coordinate referrals, maintain a resource/provider database, and prepare reports. You’ll also participate in risk management meetings, deliver presentations/trainings, and address emerging resident and facility needs.
Community Resource Coordinator Requirements:
- 1+ year professional experience developing, implementing, or evaluating programs in human services, education, sociology, or public health/insurance programs
- Bachelor’s degree in Human Services or Education (OR equivalent combination of education/experience that includes the 1+ year requirement)
- Knowledge of resources for low-income residents
- Proficiency with Microsoft Office Suite and general computer skills
- Strong writing skills (clear documentation; ability to write technical/interpretive analysis)
- Research, problem-solving, and critical-thinking skills
- Dependable vehicle + flexible schedule for regular regional travel
