Job Description

Lakeshore Talent is in search of a Licensed Practical Nurse (LPN) to join our team in partnership with a leading healthcare organization.


This is an exciting Direct Hire opportunity for a compassionate nurse who thrives on building meaningful patient relationships while delivering high-quality care in a supportive environment.


📍 Location: PACE Center – 8405 W Alameda Ave, Lakewood, CO 80226

💵 Pay Rate: $65k - $80k

🚗 Travel: Estimated 20–100 miles per day (mileage reimbursed). Coverage area includes Arvada, Littleton, Golden, and I-25 corridor.

🕒 Schedule: Sunday–Thursday, 7:00 AM – 3:30 PM

What You’ll Do

  • Visit 5–15 participants daily, primarily providing wound care and routine nursing procedures
  • Deliver general nursing care including medication administration, treatments, vaccinations, and phlebotomy
  • Educate participants and families on medications, treatments, and care plans
  • Document care within 24 hours of visits (training provided on documentation systems)
  • Collaborate with physicians, RNs, and care teams to ensure quality outcomes
  • Provide care in participant homes with compassion, respect, and attention to detail


Requirements

  • Active State-Issued LPN License
  • Current First Aid & BLS certifications (American Heart Association or Red Cross only)
  • Personal vehicle, valid driver’s license, clean driving record, and auto insurance
  • High School Diploma or GED


Preferred

  • 3+ years of experience as an LPN in a clinic or long-term care setting, ideally with geriatrics
  • Familiarity with medications commonly used in geriatric care
