Job Title:Social Services Coordinator


Location:Full-time on-site in Brooklyn, NY


Position Summary: The Social Services Coordinator plays a critical role in supporting patients across our telehealth platform by connecting them to essential community resources, behavioral health services, financial assistance, and care navigation. This individual ensures patients receive the social support they need to thrive in their care journey, especially those facing barriers such as isolation, food insecurity, housing instability, or mental health concerns.


This is an on-site position, requiring strong communication, case management, and documentation skills. The ideal candidate will have a background in social work, case coordination, or healthcare navigation in a virtual/telehealth or outpatient setting.


Key Responsibilities:


  • Conduct virtual assessments to identify patients social, emotional, and practical support needs
  • Collaborate with clinical teams (nurses, physicians, behavioral health specialists) to coordinate holistic care
  • Refer and connect patients to appropriate community resources such as:
    - Housing support
    - Food assistance programs
    - Transportation services
    - Behavioral health care
    - Financial and insurance navigation
  • Advocate for patients and help remove barriers to care continuity
  • Maintain accurate documentation of assessments, referrals, and follow-up in the electronic health record (EHR) or care coordination software
  • Track and follow up on referrals to ensure successful connection and outcomes
  • Participate in interdisciplinary case conferences and care planning discussions
  • Ensure all interactions comply with HIPAA and company privacy protocols
  • Stay informed of regional and national support programs and maintain an updated resource database

Required Qualifications:


  • 1-2 years experience in case management, medical social work, or patient advocacy
  • Bilingual fluency in Cantonese & Mandarin
  • Experience working in a healthcare or telehealth setting preferred
  • Excellent verbal, written, and virtual communication skills
  • Comfortable using care coordination tools, EMRs, and video conferencing platforms
  • Strong understanding of community-based resources, public benefits, and healthcare systems
  • High level of empathy, discretion, and organizational ability


Preferred Qualifications:

  • Experience working with vulnerable populations (elderly, uninsured, Medicaid/Medicare patients)
  • Familiarity with behavioral health and substance use services
