What You'll Need
- NY State LPN License (required)
- Minimum one year of Med/Surg experience in a hospital or long-term care facility
- NY State Driver’s License and reliable transportation
- Good computer skills for electronic documentation
- Compassionate, patient-focused attitude and strong communication skills
What You'll Do
- Visit patients in their homes to implement and evaluate care plans
- Provide skilled nursing care and emotional support to patients recovering from illness, injury, or surgery
- Document visits electronically and complete all required documentation within 24–48 hours
- Collaborate with the Coordinator of Care and other team members to ensure high-quality, personalized care
- Contribute to a positive, patient-centered care experience and support overall health outcomes
