Job Description

Job DescriptionWe are seeking dedicated and compassionate Case Managers to join our team in LA County. The Case Managers will be responsible for providing direct support to Veterans, developing individualized service plans, and coordinating resources to help clients achieve their housing and independence goals. Job overview:

  • Hiring method: Contract to Hire / Temp to Perm (6 months until permanent hire)
  • Location: 100% onsite - requires travel to Veterans' homes in West LA & LA Metro areas
  • Hours: Monday - Friday, 8am-5pm

Key Responsibilities:

  • Conduct comprehensive assessments of clients' needs and develop individualized service plans.
  • Provide case management services, including crisis intervention, counseling, and advocacy.
  • Coordinate and facilitate access to community resources, such as housing, healthcare, employment, and education.
  • Maintain accurate and up-to-date client records and documentation.
  • Collaborate with other team members and community partners to ensure holistic support for clients.
  • Advocate for clients' rights and work to remove barriers to services and support.

Qualifications:

  • 2 years of experience working in Case Management, with homelessness, or in a related field
  • Strong understanding of issues related to homelessness, poverty, and trauma.
  • Excellent communication, organizational, and problem-solving skills.
  • Must have a drivers license and reliable transportation to travel to clients' residences

Pay Details: $24.00 to $26.00 per hour
