Job Description
We are seeking a dedicated and compassionate Physical Therapist to join our outpatient team. The ideal candidate will deliver exceptional patient care by conducting thorough assessments and developing personalized treatment plans to help patients regain mobility and enhance their quality of life. This role requires collaboration with healthcare professionals and proficiency in therapeutic modalities and documentation, supporting a moderate patient caseload with the use of Electronic Health Records and therapeutic equipment.
Responsibilities
- Conduct comprehensive patient assessments to evaluate physical abilities and limitations
- Develop individualized treatment plans aligned with patient goals
- Implement therapeutic exercises and modalities to facilitate recovery
- Collaborate with healthcare professionals to ensure coordinated patient care
- Monitor patient progress and adjust treatment plans as needed
- Provide discharge planning support to ensure continuity of care
- Maintain accurate documentation using Electronic Health Records
- Educate patients on therapy procedures and home exercise programs
Required Qualifications
- Valid state licensure as a Physical Therapist
Preferred Qualifications
- Doctorate degree in Physical Therapy
- 1+ years of experience in physical therapy
- Proficiency in manual therapy techniques
- Familiarity with the functional movement system
- Understanding of medical terminology
- Strong patient communication and treatment plan development skills
- Experience with therapeutic modalities
- Demonstrated ability to collaborate within multidisciplinary teams
